摘　　要：目的探讨急性脑出血患者血清S100B蛋白浓度及其变化的临床意义。方法选取2015-01至2016-04武警某三甲医院收治的46例急性脑出血患者为研究对象,分别采用磁微粒化学发光法和电化学发光法检测血清S100B蛋白的浓度,分析手术前后该蛋白浓度的变化,同时与30例健康体检者进行对比,评价治疗前S100B蛋白浓度与美国国立卫生研究院的卒中量表(National Institute of Health Stroke Scale,NIHSS)评分、血肿量的关系。结果 (1)磁微粒化学发光法与电化学发光法的检测结果无统计学差异(t=0.716,P=0.079);(2)与对照组相比,脑出血组入院初血清S100B蛋白水平显著升高(t=5.872,P=0.004),22例接受手术治疗患者术后S100B蛋白水平较术前显著下降(t=2.716,P=0.007),且与正常对照组相比无统计学差异(t=0.087,P=0.669);(3)血清S100B水平与患者血肿量(r=0.645,P<0.01)和NIHSS评分(r=0.529,P<0.01)呈正相关。结论 S100B蛋白在急性脑出血患者中高表达,可作为早期判断急性脑出血严重程度和治疗效果的血清标志物,为脑出血的临床诊断和预后治疗提供方向。Objective The aim of this study was to investigate the clinical significance of the changes of serum S100B protein concentration in patients with acute intracerebral hemorrhage(ICH).Methods A sample of46patients with ICH that presented to a third-grade class-A hospital of PAP between January2015and April2016were selected,concentrations of serum S100B protein were detected by using magnetism-particulate-immuno-chemistry luminescence method and electrochemiluminescence assay respectively.The changes of serum S100B protein concentration before and after surgery were analyzed and compared with30healthy individuals as a control.The relationship between serum S100B protein concentration and National Institute of Health Stroke Scale(NIHSS)score and hematoma volume before treatment were evaluated respectively.Results(1)There were no significant statistical difference between the two methods in detecting serum S100B protein(t=0.716,P=0.079).(2)The concentration of serum S100B protein on admission in ICH group was significantly increased as compared to the control group(t=5.872,P=0.004).The post surgey serum S100B protein concentration of22patients who underwent surgical treatment was significantly lower than before surgery(t=2.716,P=0.007),and there was no significant difference as compared to the control group(t=0.087,P=0.669).(3)The serum S100B protein concentration was positively correlated with the hematoma volume(r=0.645,P<0.01)and NIHSS scores(r=0.529,P<0.01).Conclusions The serum S100B protein is highly expressed in patients with ICH,it can be used as an early serum marker to determine the severity and treatment effect of ICH,and as a predictor of prognosis of ICH.
