摘　　要：目的评价应用PDCA循环管理规范婴幼儿腹腔镜下内环口高位结扎术围手术期预防性应用抗菌药物的效果。方法 2013年1月~2014年12月0~3岁婴幼儿腹腔镜下内环口高位结扎术出院病历为非干预组(200例);2015年1月~2016年8月0~3岁婴幼儿腹腔镜下内环口高位结扎术出院病历为干预组(291例),应用PDCA循环管理模式进行干预。对两组围手术期预防性使用抗菌药物的合理性给予评价。结果非干预组预防性使用抗菌药物的病例为67例,预防用药比例为33.50%(67/200);干预组预防性使用抗菌药物的病例为7例,预防用药比例为2.41%(7/291),两组比较差异有统计学意义(P<0.05)。两组给药时机的合理率、24 h停药率比较差异均有统计学意义(P<0.05)。结论通过PDCA循环管理,婴幼儿腹腔镜下内环口高位结扎术围手术期预防用药比例明显降低,不仅促进该类手术围手术期抗菌药物的合理使用,同时对遏制细菌耐药、节省医疗费用和降低药物不良事件的发生等方面有着重要的作用。Objective To evaluate effect by application of PDCA circulation in management andspecification of perioperative preventative medication of antibacterial agents for infantile laparoscopic internalorifice high ligation.Methods There were discharged0~3years old infantile laparoscopic internal orifice highligation cases in January2013~December2014as non-intervention group(200cases)and discharged0~3yearsold infantile laparoscopic internal orifice high ligation cases in January2015~August2016as intervention group(291cases).PDCA circulation management mode was applied for intervention.Rationality of perioperative preventativemedication of antibacterial agents was evaluated in the two groups.Results There were67cases with preventativemedication of antibacterial agents in the non-intervention group,with preventative medication rate as33.50%(67/200).There were7cases with preventative medication of antibacterial agents in the intervention group,withpreventative medication rate as2.41%(7/291).The difference between the two groups had statistical significance(P<0.05).The differences of rational rate of medication occasion and24h drug withdrawal rate al had statisticalsignificance between the two groups(P<0.05).Conclusion Implement of PDCA circulation management canremarkably lower perioperative preventative medication rate for infantile laparoscopic internal orifice high ligation.This method can promote rational application of perioperative antibacterial agents,along with important effects forrestraining antimicrobial resistance,reducing medical cost and lowering incidence of adverse drug events.
