摘　　要：目的探讨全身联合鼓室给予地塞米松治疗重度特发性突聋的疗效及预后影响其因素,以期为临床治疗提供依据。方法选取80例重度特发性突聋患者,随机均分为研究组和对照组,各40例。对照组给予全身激素治疗,研究组给予全身联合鼓室给予地塞米松治疗,综合比较两组治疗总有效率、不同频率听阈改善情况,并分析伴随耳鸣和眩晕症状对预后的影响,采用SPSS20.0统计学软件进行数据分析。结果研究组治疗总有效率为75.0%,对照组治疗总有效率为72.5%,治疗总有效率比较差异无统计学意义;两组各频率听阈改善情况比较差异无统计学意义;研究组和对照组耳鸣症状治疗有效率(71.43%vs.70.00%)及眩晕症状治疗有效率(53.33%vs.64.28%)比较差异均无统计学意义。结论全身联合鼓室给予地塞米松治疗与单纯全身给药疗效相当,耳鸣及眩晕症状对患者预后无显著影响。Objective To investigate effects of intratympanic and systemic dexamthasone for ISSNHL and the prognostic factors,in order to provide the basis for clinical treatment.Methods80cases of patients with ISSNHL were average randomly divided into research group and control group.Total effective rate,improvement of hearing thresholds at different frequencies and prognostic factors were compared.The data were analyzed by SPSS20.0statistical software.Results The total effective rate of the treatment group was75%,the total effective rate of the control group was72.5%,and the total effective rate was not statistically significant.There was no obvious difference of improvement of the frequency threshold between research group and control group.There were no significant differences in tinnitus symptoms treatment efficiency(71.43%vs.70.00%)and vertigo symptoms treatment efficiency(53.33%vs.64.28%)between research group and control group.Conclusion There is no significant difference in effect for ISSNHL between intratympanic combined with systemic dexamthasone and systemic dexamethasone alone.Tinnitus or dizziness had no obvious impact on the prognosis.
