微创经皮并结合伤椎椎弓根螺钉固定治疗胸腰椎骨折的效果体会

摘　　要：目的探讨微创经皮并结合伤椎椎弓根螺钉固定治疗胸腰椎骨折的效果。方法 48例胸腰椎骨折患者,随机分为实验组和常规组,每组24例。常规组患者采用开放性椎弓螺钉内固定治疗,实验组患者采用微创经皮并结合伤椎椎弓根螺钉固定治疗,对比两组患者的临床疗效。结果实验组患者术中出血量为(83.6±65.6)ml、切口长度为(9.1±1.4)cm、手术时间为(112.3±20.6)min、术后引流量为(5.3±4.6)ml、疼痛评分为(2.6±1.6)分、住院时间为(9.3±2.6)d,均优于常规组的(369.3±62.3)ml、(15.6±2.3)cm、(125.322.6)min、(220.3±66.3)ml、(4.8±2.4)分、(15.3±2.9)d,差异均具有统计学意义(P<0.05)。结论针对胸腰椎骨折的患者采用微创经皮并结合伤椎椎弓根螺钉固定治疗效果显著,可有效改善患者预后,值得临床广泛推广应用。Objective To investigate effect by minimally invasive percutaneous combined with injuredvertebral pedicle screw fixation in the treatment of thoracolumbar vertebral fracture.Methods A total of48patients with thoracolumbar vertebral fracture were randomly divided into experimental group and conventionalgroup,with24cases in each group.The conventional group received open pedicle screw internal fixation fortreatment,and the experimental group received minimally invasive percutaneous combined with injured vertebralpedicle screw fixation for treatment.Clinical effects were compared between the two groups.Results Theexperimental group had intraoperative bleeding volume as(83.6±65.6)ml,incision length as(9.1±1.4)cm,operationtime as(112.3±20.6)min,and postoperative drainage volume as(5.3±4.6)ml,pain score as(2.6±1.6)points andhospital stay time as(9.3±2.6)d,which were all better than(369.3±62.3)ml,(15.6±2.3)cm,(125.3±22.6)min,(220.3±66.3)ml,(4.8±2.4)points and(15.3±2.9)d in the conventional group,and their difference had statistical significance(P<0.05).Conclusion Implement of minimally invasive percutaneous combined withinjured vertebral pedicle screw fixation in the treatment of thoracolumbar vertebral fracture patients showsexcellent curative effect.This method can effectively improve prognosis in patients,and it is worth widely clinicalpromotion and application.
