子宫动脉与脐动脉血流分析在预测妊高征发生的应用研究

摘　　要：目的:通过对子宫动脉与脐动脉的多普勒超声检测,分析其血流频谱与妊娠高血压病之间的关系,预测妊娠高血压综合征的发生。方法:选取在我院接受产前检查且16~20孕周的孕妇500例,所有孕妇均行彩色多普勒超声检测,检测子宫动脉、脐动脉的血流频谱,包括收舒末期最大血流速度PS、舒张末期最大血流速度ED、收舒末期最大血流速度/舒张末期最大血流速度之比S/D、阻力指数RI及搏动指数PI,观察记录检测结果。28孕周后对所有患者行彩色多普勒超声检测,监测子宫-胎盘动脉、脐动脉的血流,血压、血糖、小便常规检查等,观察孕妇出现妊高症的概率。结果:28孕周后有100例孕妇出现妊娠高血压综合征,发病概率为20%,其子宫动脉及胎儿脐动脉的血流指标均明显高于正常妊娠的孕妇,P<0.05,差异具有统计学意义。结论:利用彩色多普勒超声检测监测孕妇子宫动脉与脐动脉血流指标变化情况可准确对妊高症进行判断,对预测妊高症的发生、减少并发症的发生、提高母婴生存率具有重要的作用,值得在临床上进一步推广应用。Objective:To analyze the relationship between blood flow spectrum and gestational hypertension and predict theoccurrence of pregnancy-induced hypertension(PIH)syndrome through Doppler ultrasound detection of uterine artery and umbilical artery.Methods:500pregnant women with16~20gestational weeks accepted color Doppler ultrasonic detection to examine the blood flowspectrum of uterine artery and umbilical artery,including the maximum blood flow velocity at end systole stage(PS),the maximum bloodflow velocity at end diastolic stage(ED),the ratio of PS to ED(S/D),resistance index(RI)and pulsation index(PI),and the test resultswere observed and recorded to observe the risk of PIH.Results:Pregnancy-induced hypertension syndrome was found in100cases on28gestational weeks,with an incidence of20%.Their uterine artery and fetal umbilical artery blood flow index were significantly higher thanthat of normal pregnant women and the difference was statistically significant(P<0.05).Conclusion:Color Doppler ultrasonic detection wasused to monitor the changes of uterine artery and umbilical artery blood flow index in pregnant women to accurately judge PIH.It plays animportant role in predicting the occurrence of PIH,reducing complications and improving maternal and child survival.
