产妇精神因素对分娩方式及产后出血影响的研究

摘　　要：目的:根据产妇的精神因素对分娩的方式及产后出血的影响进行分析研究。方法:选取近两个月在我附属医院待产的产妇90例,根据汉密顿焦虑表和抑郁量表来进行评定,根据分娩方式的不同分为三组,A组产妇采取剖宫产的分娩方式;B组产妇采取产钳的分娩方式;C组产妇采取自然分娩的方式,对患者的分娩方式及产后出血的情况进行分析。结果:①A组产妇焦虑和抑郁共占43.3%,B组产妇焦虑和抑郁共占50.0%,C组产妇焦虑和抑郁共占16.6%,A组和B组产妇与C组产妇相比较,较焦虑评分和抑郁评分有明显的差别;在精神状态与分娩方式的评分比较中,A组和B组产妇与C组产妇比较,数据具有明显的差异;产后出血组和无出血组比较,焦虑评分和抑郁评分有明显的差别,数据差异具有统计学意义(P<0.05);②在三组产妇精神状态评分的比较中,A组产妇和B组产妇较焦虑评分和抑郁评分没有明显的差异;在精神状态与分娩方式的评分比较中,A组产妇和B组产妇比较没有差异,不存在统计学意义(P>0.05)。结论:通过此次研究发现,焦虑及抑郁是导致产妇产后出血的重要因素之一,对于患有焦虑或抑郁的产妇医护人员及家属应给予心理辅导,使其对分娩过程有充分的了解及认识,并使用科学的手段帮助产妇减少阵痛及分娩时的痛苦,对生产后出现的并发症给予及时正确的处理方法,增加产妇分娩的信心,以减少产妇产后出血的发生。Objective:According to the maternal factors of mental factors of delivery and postpartum hemorrhage were analyzed.Methods:90mothers were selected and divided into three groups according to the Hamilton Anxiety Scale and Depression Rating Scale.Group A adopted the cesarean section mode;Group B,the placenta to take the production mode of production;C group took the naturalway of delivery.Results:The anxiety and depression were43.3%in group A,50.0%in group B and16.6%in group C.The anxiety anddepression of group A and group B were significantly higher than those of group C.Compared with the postpartum hemorrhage group,theanxiety scores and the scores(P<0.05).There was no significant difference between the two groups in the scores of anxiety and depressionin the comparison of the three groups of maternal mental state scores.In group A and group B,there was no significant difference in scoresof anxiety and depression,(P>0.05).Conclusion:Anxiety and depression is an important factor in maternal postpartum hemorrhage,andpsychological counseling is important to reduce maternal postpartum hemorrhage.
