糖尿病肾病患者尿胱抑素C和α1-微球蛋白水平变化研究

摘　　要：目的:研究糖尿病肾病患者尿胱抑素C(CysC)和α1-微球蛋白(α1-MG)水平变化。方法:选取我院2012年7月-2014年7月糖尿病肾病患者52例和单纯糖尿病患者40例,其中糖尿病肾病分为微量白蛋白尿亚组24例和临床蛋白尿亚组28例,再选取同期于我院体检的健康人40例设置为对照组,比较各组血清CysC、α1-MG和尿CysC、α1-MG的水平及异常率。结果:糖尿病肾病组患者血清CysC、α1-MG及尿CysC、α1-MG水平较对照组和单纯糖尿病组显著较高(P<0.05);微量清蛋白尿亚组血清CysC、α1-MG及尿CysC、α1-MG水平较临床蛋白尿亚组显著较低(P<0.05);糖尿病肾病组CysC、α1-MG及尿CysC、α1-MG异常率显著高于对照组和单纯糖尿病组(P<0.05)。结论:糖尿病肾病患者尿CysC和α1-MG水平较单纯糖尿病患者及对照组显著较高,在糖尿病肾病的早期诊断中具有重要的临床价值,值得应用推广。Objective:To study the changes of urinary cystatin C andα1-microglobulin in patients with diabetic nephropathy.Methods:52patients with diabetic nephropathy and40patients with diabetes mellitus were selected from July2012to July2014.Patientswith diabetic nephropathy were divided into the microalbuminuria group(24cases)and clinical proteinuria group(28cases).40casesof healthy people in our hospital were selected as the control group,and the levels of serum Cys C,α1-MG and urinary Cys C andα1-MG were compared.Results:The levels of serum Cys C,α1-MG and urinary Cys C andα1-MG in the diabetic nephropathy group weresignificantly higher(P<0.05).The abnormal rate of Cys C,α1-MG and urinary Cys C andα1-MG in diabetic nephropathy group weresignificantly higher(P<0.05),and the level of urinary Cys C andα1-MG was significantly lower(P<0.05).Conclusion:The levels ofurinary Cys C andα1-MG in patients with diabetic nephropathy are significantly higher than those in patients with diabetes mellitus andhealthy people.
