减低剂量免疫球蛋白联合激素治疗原发免疫性血小板减少症临床分析

摘　　要：目的探讨减低剂量免疫球蛋白联合激素治疗原发免疫性血小板减少症的临床疗效。方法 40例患者随机分为两组,各20例,治疗组为免疫球蛋白0.2 g/(kg·d)×5 d,静脉滴注,甲泼尼龙1 mg/(kg·d),静脉滴注。对照组为甲泼尼龙1 mg/(kg·d),静脉滴注。对两组的临床疗效、出血症状控制时间及血小板上升变化进行观察和比较。结果治疗组总有效率95.00%(19/20),对照组总有效率70.00%(14/20),治疗组出血症状控制时间(1.61±0.62)d,对照组(2.90±1.20)d,治疗组第3天血小板上升至(44.90±17.78)×109/L,对照组(28.20±9.12)×109/L,治疗组的出血症状控制时间、血小板上升时间及恢复时间明显缩短,总有效率明显升高,差异有统计学意义(P<0.05)。结论减低剂量免疫球蛋白联合激素治疗原发免疫性血小板减少症能够迅速控制出血症状,明显缩短血小板上升时间,减少了患者发生严重出血并发症的危险,且减轻了患者的医疗费用。Objective To investigate the clinical efficacy of reduced dose immunoglobulin combined with hormone in the treatment of ITP.Methods40patients were randomly divided into2groups,each group of20cases.The treatment group have Intravenous immunoglobulin0.2g/(kg·d)×5d and methylprednisolone1mg/(kg·d).The control group have intravenous methylprednisolone1mg/(kg·d).The clinical efficacy,the control time of bleeding symptoms and the change of platelet were observed and compared between the two groups.Results The total efficiency of treatment group was95%(19/20),the control group total effective rate was70%(14/20),the treatment group bleeding control time(1.61±0.62)days,the control group of(2.90±1.20)days,the treatment group third days of platelet that goes up to(44.90±17.78)×109/L,the control group(28.20±9.12)×109/L.In the treatment group,the time of bleeding symptom control,the time of platelet rise and recovery time were significantly shortened.The total effective rate was significantly increased,the difference was statistically significant(P<0.05).Conclusion Reduced dose immunoglobulin combined with hormone treatment of ITP can rapidly control bleeding symptoms,shorten significantly the time of platelet rise,reduce the risk of serious bleeding complications,and reduce the cost of medical treatment.
