我中心2011-2014年抗艾滋病毒药物应用分析

摘　　要：目的:为临床合理使用及管理抗艾滋病毒药物提供参考,并为职业暴露制订免费用药政策提供依据。方法:采用回顾性研究方法,按照世界卫生组织推荐的限定日剂量(DDD)法进行计算,对我中心2011-2014年使用抗艾滋病毒药物的销售金额、用药频度(DDDs)、日均费用(DDC)和序号比(B/A)等进行分析和评价。结果:我中心2011-2014年抗艾滋病毒药物销售金额呈先升高后降低的趋势,由2011年的512 112元上升至2012年的650 854元,然后逐年降低至2014年的496 473元;DDDs由2011年的35 932逐年下降至2014年的16 363。核苷酸类逆转录酶抑制剂的销售金额在2012-2014均列第1位,DDDs在2011、2013、2014年均列第1位。齐多夫定片的DDDs在2013、2014年列第1位,唐草片的DDDs在2011-2014年均列前3位。富马酸替诺福韦二吡呋酯片、齐多拉米双夫定片、恩曲他滨替诺福韦片、洛匹那韦利托那韦片的DDC较高,在46元以上;齐多夫定片、司他夫定片、奈韦拉平片的DDC较低,在6元以下。富马酸替诺福韦二吡呋酯片、司他夫定片和依非韦伦片的B/A值4年均在0.5~1.5,唐草片在2013、2014年的B/A=1,社会效益和经济效益同步性较好。结论:我中心抗艾滋病毒药物临床使用基本合理,药物应用与职业暴露预防用药基本相符,中药制剂的应用符合临床用药需求。OBJECTIVE:To provide reference for clinical rational use and management of anti-HIV drugs,and to provide referencebasis for occupational exposure to develop free drug policy.METHODS:Using retrospective study method,according toDDD calculation method recommended by WHO,consumption sum,DDDs,DDC and B/A of anti-HIV drugs in our center during2011-2014were analyzed and evaluated.RESULTS:The consumption sum of anti-HIV drugs increased firstly and decreased later,increasing from512112yuan in2011to650854yuan in2012,decreasing to496473yuan in2014.DDDs of anti-HIV drugs inour center during2011-2014decreased from35932in2011,to16363in2014,showing decreasing tendency.The consumptionsum of nucleotide reverse transcriptase inhibitors during2012-2014took up the first place,and DDDs of nucleotide reverse transcriptaseinhibitors took up the first place in2011,2013,2014.DDDs of Zidovudine tablets took up the first place in2013,2014,and that of Tangcao tablets took up top3places during2011-2014.DDC of Tenofour disoproxil fumarate tablets,Zidovudine and lamivudine tablets,Emtricitabine and tenofovir tablets,Lopinavir and ritonavir tablets were in high level,being more than46yuan.DDC of Zidovudine tablets,Stavadine tablets and Nevirapine tablets were in low level,being lower than6yuan.B/A of Tenofovirdisoproxil fumarate tablets,Stavudine tablets and Efavirenz tablets ranged0.5-1.5in4years and B/A of Tangcao tablets was equalto1,showing good synchronism of social and economical benefits.CONCLUSIONS:Clinical application of anti-HIV drugs in ourcenter is rational basically,and the use of drug is basically consistent with the occupational exposure prevention.The use of TCMpreparation is basically in line with the requirements of drug use in the clinic.
