超声诊断在先天性肠旋转不良合并中肠扭转患儿中的临床意义

摘　　要：目的分析超声诊断在先天性肠旋转不良合并中肠扭转患儿中的临床意义。方法选取上消化道梗阻手术患儿163例,回顾性分析其临床资料、术前超声检查结果及X线造影检测结果,并将超声检查结果与消化道造影检查结果对比分析,使用统计软件进行卡方分析。结果在肠旋转不良的检测中,超声诊断的敏感度为90.00%,特异度为90.70%,准确率为90.18%,假阴性率为10.00%,假阳性率为9.30%,超声检查与手术结果具有相当满意的一致程度(Kappa=0.762);在肠旋转不良合并中肠扭转患儿检测中,超声诊断敏感度为93.83%,特异性为94.87%,准确率为94.17%,假阴性率为6.17%,假阳性率为5.13%;超声检查与手术结果具有相当满意的一致程度(Kappa=0.870);超声检查在先天性肠旋转不良合并中肠扭转患儿中的确诊率明显高于X线造影检查,差异具有统计学意义(P<0.05)。结论临床怀疑肠旋转不良合并中肠扭转患儿,术前通过超声和X线造影检查均可进行诊断,而超声检查对于先天性肠旋转不良合并中肠扭转患儿具有更高的诊断价值,为临床治疗提供重要参考依据。Objective To analyze the clinical significance of ultrasonic diagnosis in children with bowel volvulus with the merger of rotatingadverse.Methods The subjects were163cases of patients with digestive tract obstruction in our hospital,retrospective analysis the clinical dataand preoperative ultrasound examination and X-ray imaging detection results.and analysis the difference between the preoperative ultrasound examinationand X-ray imaging detection results.Results In the intestinal rotation poor detection,the sensitivity of the ultrasonic diagnosis was90.00%,the specificity was90.70%,the accuracy is90.18%,the false negative rate was10.00%,false positive rate was9.30%,ultrasound is consistent withthe surgical results is quite satisfactory(kappa=0.762);in intestinal volvulus with the merger of rotating bad tests,ultrasonic diagnostic sensitivitywas93.83%,specificity was94.87%,the accuracy was94.17%,the false negative rate was6.17%,false positive rate was5.13%,ultrasound is consistentwith the surgical results is quite satisfactory(kappa=0.870);ultrasound in congenital intestinal volvulus with the merger of bad of diagnosis rateis significantly higher than X-ray angiography examination,the difference is statistically significant(P<0.05).Conclusion Clinical suspected intestinalvolvulus with the merger of rotating adverse,ultrasonic testing and x ray contrast radiography testing all can diagnose,ultrasound examinationfor congenital intestinal volvulus with the merger of rotating bad has higher diagnostic value,provide important reference basis for clinical treatment.
