中度肥大乳房男性化手术策略及效果

摘　　要：目的:探讨女复男易性病患者中度肥大乳房的男性化手术策略和效果。方法:选择40例乳房中度肥大的易性病患者,采用二步法完成乳房男性化手术,即先经乳晕上切口切除乳腺和外周脂肪组织,6个月后再经乳晕外双环法去除多余胸部褶皱皮肤并调整乳头乳晕大小和位置。结果:40例患者术后胸部平整,乳头乳晕全部成活,乳晕大小、位置合适,切口瘢痕不明显;部分患者术后乳头感觉轻度麻木,6个月后基本恢复正常。结论:依照本策略完成中度肥大乳房患者的男性化手术效果满意,在提升乳头乳晕位置的同时去除多余褶皱皮肤,瘢痕小,恢复快,是针对中度肥大乳房患者较为实用的男性化手术方法。Objective To investigate the indications,methods and results of the two-stage mastectomy of female-to-male patientswith moderate hypertrophy breasts.Methods Two-stage mastectomes were performed in40female-to-male transsexuals sinceJanuary2014using the following techniques.In the first stage operation,the breast tissue was removed through the curvedincision in superior margin of areola;6months later.Adjust the areola size and Repair the skin folds by removing of excessskin tissue though the incision exterior margin of areola.Results The chests were similar to the male in all40cases.Thenipple areola survived well and the size of areola was appropriate,the incision scars were not obvious.A few patients felt mildnumbness in nipple and areola area and recovery after6months.Conclusion At the same time of correcting of nipple position,excess skin was removed according to this strategy,the effect of female-to-male mastectomy was satisfied also with less surgicaltrauma and less obvious incision scar.
