妇产科术后术区皮肤增生性瘢痕形成因素分析

摘　　要：目的:分析妇产科术后术区皮肤增生性瘢痕形成的影响因素。方法:选取2015年8月-2016年8月于本院妇产科行手术治疗的70例患者作为研究对象,依据术后术区皮肤增生性瘢痕发生与否分为未发组50例及增生性瘢痕组20例,采用Logistic回归分析模型对诱发术后术区皮肤增生性瘢痕的独立影响因素进行分析。结果:两组患者年龄、手术类型相比较,差异无统计学意义(P>0.05);切口类型、胶原含量、结缔组织生长因子、胸苷磷酸化酶、皮肤色素沉着、美容修复、感染、切口缝合方式差异有统计学意义(P<0.05);Logistic回归分析结果提示,色素沉着、切口缝合方式、美容修复为妇产科术后术区皮肤增生性瘢痕的独立影响因素(OR:2.890、3.554、4.420,95%CI:1.650~4.250、1.745~5.878、1.880~6.540,P<0.05)。结论:色素沉着、切口缝合方式、美容修复为妇产科术后术区皮肤增生性瘢痕的独立影响因素,去除色素沉着、采取皮内缝合、积极开展美容修复将有助于阻断增生性瘢痕的形成。Objective To analyze the factors influencing the formation of hypertrophic scar in the postoperative area ofObstetrics and gynecology.Methods From August2015to August2016,70cases of Obstetrics and gynecology in ourhospital patients as the research object,on the basis of whether postoperative skin scar is not divided into group50cases andhypertrophic scar group20cases,using Logistic model to analyze the independent factors of postoperative skin scar inducedpostoperative regression analysis.Results Two groups of patients with age,type of surgery in comparison,no statisticallysignificant difference(P>0.05).Statistically significant types of incision,collagen content,connective tissue growth factor,thymidine phosphorylase,skin pigmentation,cosmetic repair,infection,incision suture difference(P<0.05).The results ofLogistic regression analysis showed beauty,pigmentation,method of suture and repair are independent prognostic factors ofpostoperative skin hypertrophic scar after gynecologic surgery(OR:2.890,3.554,4.420.95%CI:1.650-4.250,1.745-5.878,1.880-6.540,P<0.05).Conclusion Pigmentation,cosmetic incision suture method and repair are independent prognostic factors ofpostoperative skin hypertrophic scar after gynecologic surgery,removal of pigmentation,take intradermal suture and activelycarry out the cosmetic repair form will help block the hypertrophic scar.
