摘　　要：目的:探讨N末端脑利钠肽(NT-proBNP)对重度烧伤患者心功能评价的临床意义。方法:选取在我院重症监护室(ICU)进行治疗的重度烧伤患者78例,按是否有心力衰竭发生分为心衰组(31例)和非心衰组(47例)。比较2组不同时间NT-proBNP水平变化情况、心功能指标、相关指标及病死率情况。结果:心衰组NT-proBNP、肌钙蛋白(CTnT)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、每博输出量指数(SVI)、心脏指数(CI)、中心静脉压(CVP)水平及急性生理学及慢性病健康状况(APACHE Ⅱ)评分较非心衰组明显提高,差异具有统计学意义(P<0.05)心衰组平均动脉压(MAP)较非心衰组明显降低,差异具有统计学意义(P<0.05);心衰组病死率较非心衰组明显升高,差异具有统计学意义(P<0.05)。结论:心力衰竭是重度烧伤患者常见的并发症,且病死率较高,NT-proBNP可有效评估病情及预后情况。Objective To explore the clinical significance of N?terminal pro?brain natriuretic peptide(NT?proBNP)in the evaluation of cardiac function in patients with severe burns.Methods A total of78patients withsevere burns in intensive care unit(ICU)in our hospital were selected.Patients were divided into a heart failuregroup(n=31)and a non?heart failure group(n=47)according to the heart failure complication condition.Thechanges of NT?proBNP level in different time,heart function index,relevant index and mortality were comparedbetween the two groups.Results The levels of NT?proBNP,troponin(CTnT),creatine kinase(CK),creatinekinase isoenzyme(CK?MB),stroke volume index(SVI),cardiac index(CI)and central venous pressure(CVP),and the score of acute physiologic and chronic health conditions(APACHE II)in the heart failure groupwere shown to be significantly increased than those of the non?heart failure group(P<0.05).The mean arterialpressure(MAP)in the heart failure group was significantly lower than the non?heart failure group(P<0.05).Mortality rate in the heart failure group was shown to be significantly higher than the non?heart failure group(P<0.05).Conclusion Heart failure is a common complication in patients with severe burns.The mortality ratein the population with heart failure is relatively high,for which NT?proBNP can effectively assess the conditionsand prognosis.
