摘  要:目的:了解本地流感嗜血杆菌在儿童呼吸道疾病中的感染情况及耐药性,为临床合理使用抗菌药物提供参考。方法:收治上呼吸道感染患者576例,采用无菌负压吸引法采集新鲜痰液,进行流感嗜血杆菌培养和鉴定,分离的流感嗜血杆菌用头孢硝噻吩纸片法检测β-内酰胺酶,药敏用ATB试剂盒进行测定。结果:在576例标本中,共分离出100株流感嗜血杆菌,阳性率17.4%;在100株流感嗜血杆菌中,检出71株β-内酰胺酶阳性株,产酶率71%。耐药性监测中,复方新诺明耐药率最高(88.0%),头孢噻肟和氧氟沙星耐药率均为0。结论:流感嗜血杆菌是本地区儿童常见呼吸道感染菌,在治疗上不宜选用复方新诺明、氨苄西林和一代头孢菌素,多重耐药现象应引起重视。临床治疗可优先考虑阿莫西林/棒酸和头孢噻肟。Objective:To investigate the local infection status and drug resistance of haemophilus influenzae in children withrespiratory tract diseases,to provide reference for rational use of antibiotics in clinical practice.Methods:576patients with upperrespiratory tract infection were selected.We collected fresh sputum with sterile negative pressure suction method to culture andidentify haemophilus influenza.The isolated haemophilus influenzae was tested for beta lactamase using the cefoxitin disk method.Drug sensitivity was determined by ATB kit.Results:A total of100strains of haemophilus influenzae were isolated from576specimens,the positive rate was17.4%.Of the100strains haemophilus influenzae,71strains were beta lactamase positive and theenzyme production rate was71%.In drug resistance surveillance,the drug resistance rate of sulfamethoxazole was thehighest(88%),and the rates of resistance to cefotaxime and ofloxacin were zero.Conclusion:Haemophilus influenzae was a commonrespiratory tract infection bacteria in children in this region.In the treatment,we should not choose compound sulfamethoxazole,ampicillin and first generation cephalosporin,and we should pay attention to multi drug resistance.Amoxicillin/clavulanic acid andcefotaxime can be given priority in clinical treatment.
