异丙酚复合芬太尼或瑞芬太尼靶控静脉麻醉与静吸复合麻醉的比较

摘　　要：目的探究异丙酚复合芬太尼或瑞芬太尼靶控静脉麻醉的麻醉效果，并与静吸复合麻醉的麻醉效果进行比较。方法选择ASAⅠ-Ⅱ级患者90例，依照不同的麻醉方式随机分成A(静吸复合麻醉)组，B(异丙酚复合芬太尼麻醉)组，C(异丙酚复合瑞芬太尼麻醉)组，每组患者30例，观察比较3组患者拔管时及拔管后疼痛评分、清醒时间、定向力恢复时间、拔管时间；观察比较恶心、呕吐等不良反应发生情况及患者术中知晓情况等。结果3组患者术后清醒时间、定向力恢复时间、拔管时间比较差异无统计学意义(P>0.05)；3组患者在拔管时、拔管后1h、拔管后3h比较，B、C组患者疼痛感显著低于A组，差异比较具有统计学意义(P<0.05)。A、B、C3组患者不良反应发生率比较差异比较无统计学意义(P>0.05)。结论异丙酚复合芬太尼或瑞芬太尼把控静脉麻醉的效果更加稳定，患者疼痛感更少，不良反应发生率更低，具有很好的临床推广意义。Objective:To explore propofol combined with fentanyl or remifentanil target controlled intravenous anesthesia anesthesia effect,and compared with the general anesthesia effect of anesthesia.Methods:ASA I-II patients90cases,according to the different ways of anesthesia were divided into A(general anesthesia)group B(propofol fentanyl anesthesia)group C(propofol remifentanil anesthesia)group,30cases in each group were observed and compared between the three groups during extubation and after extubation pain score,recovery time,recovery time,extubation time were compared;nausea,vomiting and other adverse reactions and the incidence of intraoperative awareness in patients condition.Results:Patients the three groups after surgery recovery time,recovery time,extubation time was no significant difference(P>0.05);three patients at extubation,1h after extubation,3h after extubation,B,C two groups of patients with pain was significantly lower than that of A group,the difference was statistically significant(P<0.05).B,C two groups of patients with adverse reaction rate was significantly lower than that in A group,the difference was statistically significant(P<0.05).Conclusion:Propofol combined with remifentanil or fentanyl intravenous anesthesia to control effect is more stable,less pain of patients,the incidence of adverse reaction is lower,which has clinical significance good.
