不同麻醉方式在小儿下腹部手术中的应用效果观察

摘　　要：目的:探讨七氟醚+丙泊酚联合骶管阻滞麻醉和七氟醚联合骶管阻滞麻醉在小儿下腹部手术的应用效果。方法:收治行下腹部手术患儿78例,随机分为对照组和试验组。对照组采取七氟醚联合骶管阻滞麻醉,试验组在对照组基础上联合使用单次骶管阻滞复合静脉持续泵注丙泊酚麻醉,比较两组麻醉效果。结果:试验组麻醉诱导时间及苏醒时间均明显短于对照组(P<0.05)。与术前(T1)相比,试验组T2、T3、T4的MAP、HR均无明显改变(P>0.05),而对照组各时间节点MAP、HR均有一定程度升高(P<0.05)。试验组不良反应发生率低于对照组(P<0.05)。结论:七氟醚+丙泊酚联合骶管阻滞麻醉用于小儿下腹部手术的麻醉效果好,患者苏醒快,不良反应发生率低。Objective:To explore the application effect of sevoflurane and propofol combined with caudal block anesthesia andsevoflurane combined with caudal block anesthesia on lower abdominal operation in children.Methods:78cases of children withlower abdominal operation were selected.They were randomly divided into the control group and the experimental group.Thecontrol group was given sevoflurane combined with caudal block anesthesia.The experimental group was given single caudal blockcombined with intravenous continuous infusion of propofol anesthesia on the basis of the control group.We compared theanesthetic effects between two groups.Results:The anesthesia induction time and recovery time in the experimental group weresignificantly shorter than those in the control group(P<0.05).Compared with before operation(T1),there were no significantchanges in MAP and HR in the experimental group at T2,T3,T4(P>0.05),while the MAP and HR in the control group wereincreased to some extent at each time point(P<0.05).The incidence of adverse reactions in the experimental group was lower thanthe control group(P<0.05).Conclusion:The anesthetic effect of sevoflurane and propofol combined with caudal block anesthesiaon lower abdominal operation in children was good,the patients were waking up fast,and the incidence of adverse reactions was low.
