摘　　要：目的:对静脉用药调配中心(Pharmacy Intravenous Admixture Services,PIVAS)发现的不合理医嘱进行分析并干预,以促进临床合理用药。方法:调取静配中心2014年10—2016年10月和2016年11月—2017年4月各科全部医嘱,对不合理医嘱进行分析。结果:2014年10—2016年10月166788份医嘱中,不合理医嘱2835份,占1.7%;2016年11月—2017年4月共31878份医嘱,其中不合理医嘱217,占0.7%。药师定期对不合理医嘱进行总结,主动去临床科室与医生进行沟通,减少了临床不合理用药情况。结论:我院各科室医嘱存在一定的不合理现象,审方药师可以有效拦截不合理医嘱,促进临床合理用药。Objective:To analyze the irrational medical orders found in the PIVAS,in order to promote clinical rational drug use.Methods:All medical orders of clinical departments from2014.10to2016.10and from2016.11to2017.4in the PIVAS were transferred,unreasonable medical orders were analyzed.Results:There were2835unreasonable medical orders in166788from2014.10to2016.10,accounting for1.7%;there were217unreasonable medical orders in31878from2016.11to2017.4,accounting for0.7%.Pharmacists regularly sum up the irrational medical orders,and actively go to clinical departments to communicate with doctors,and reduced the clinical irrational use of drugs.Conclusion:There are some unreasonable phenomenon in our hospital,the pharmacist can effectively block the unreasonable medical advice,and promote the clinical rational use of drugs.
