摘　　要：目的:分析来氟米特联合甲泼尼龙对狼疮肾炎的诱导效果。方法:选择2014年1月—2016年12月76例狼疮肾炎患者,根据随机数字表分组,各38例。对照组采用环磷酰胺联合甲泼尼龙进行治疗,治疗组采用来氟米特联合甲泼尼龙进行治疗。比较两组狼疮肾炎治疗总有效率,干预前后患者补体C3、抗核抗体ANA阳性数、抗ds-DNA阳性数的差异,干预前后患者血肌酐、尿红细胞数、24小时尿蛋白的差异,不良反应发生率的差异。结果:治疗组狼疮肾炎治疗总有效率高于对照组,P<0.05;干预前两组补体C3、抗核抗体ANA阳性数、抗ds-DNA阳性数相近,P>0.05;干预后治疗组补体C3、抗核抗体ANA阳性数、抗ds-DNA阳性数优于对照组,P<0.05。干预前两组血肌酐、尿红细胞、24小时尿蛋白相近,P>0.05;干预后治疗组尿红细胞、24小时尿蛋白减少优于对照组,P<0.05。治疗组不良反应率较对照组低。结论:来氟米特联合甲泼尼龙对狼疮肾炎的诱导效果确切,可有效改善临床各项指标,安全性高,对患者预后良好,值得推广应用。Objective:To study the effect of leflunomide combined with methylprednisolone on lupus nephritis.Methods:A total of38patients with lupus nephritis were selected from January2014to December2016,according to a random number table.The control group was treated with cyclophosphamide combined with methylprednisolone.The treatment group was treated with leflunomide combined with methylprednisolone.Comparison of the two groups of patients with lupus nephritis treatment of total effective rate;before and after treatment of patients with complement C3,anti-nuclear antibody ANA positive number of anti-ds-DNA positive number of differences;before and after treatment of patients with serum creatinine,erythrocyte count,24-hour urine protein differences;Reaction rate difference.Results:The total effective rate of treatment group was higher than that of the control group(P<0.05).The positive rate of anti-nuclear antibody ANA and anti-ds-DNA were similar in the two groups before treatment(P>0.05);C3,anti-nuclear antibody ANA positive number,anti-ds-DNA positive number better than the control group(P<0.05).The levels of serum creatinine and erythrocyte in the two groups were similar to those in the control group(P<0.05).The urinary protein was significantly lower in the treatment group than in the control group(P<0.05).The adverse reaction rate was lower in the treatment group than in the control group.Conclusion:Leflunomide combined with methylprednisolone has obvious effect on the induction of lupus nephritis,which can effectively improve the clinical indexes,high safety,good prognosis and worthy of popularization and application.
