糖皮质激素在痛风石围手术期的临床疗效分析

摘　　要：目的观察痛风石患者围手术期应用药物治疗的有效性,寻求提高切口愈合率,减少术后并发症的方法。方法回顾性分析右江民族医学院附属医院2010年1月-2015年6月明确诊断痛风石病例73例。全部患者均采用手术清除痛风石,按围手术期药物治疗方法不同,分为糖皮质激素组、秋水仙碱组和抗生素组。观察术后发热率、胃肠道反应率及术后2周切口愈合率,并进行比较。结果糖皮质激素组疼痛缓解率明显、胃肠道反应率低,糖皮质激素组切口愈合率95%。结论痛风石围手术期合理应用糖皮质激素,可有效抑制痛风发作,促进切口愈合,缓解发热等全身症状及胃肠道反应。Objective To observe the effectiveness of perioperative drug therapy in patients with tophi,and toseek a method to raise the rate of wound healing and reduce postoperative complications.Methods A retrospectiveanalysis was carried out in73cases diagnosed as tophi in the Affiliated Hospital of Youjiang Medical College ForNationalities from January2010to June2015.All patients were treated with surgical removal of tophi,and dividedinto glucocorticoid group,colchicine group and antibiotic group according to different perioperative drug treatment.The rates of postoperative fever and gastrointestinal reactions,and the rate of wound healing2w after operation werecompared.Results In the glucocorticoid group,the pain was significantly relieved,and the gastrointestinal reactionrate was low,the wound healing rate was95%,which were better than the other groups.Conclusions Reasonableapplication of glucocorticoids in the perioperative period can effectively inhibit the gout attack,promote woundhealing,relieve fever and other systemic symptoms and gastrointestinal reactions.
