摘 要:目的探讨声触诊组织定量(VTQ)技术在慢性肾脏病(CKD)分期中的应用价值。方法随机选取不同临床分期的CKD患者96例及健康志愿者(对照组)45例行VTQ检查,测量肾实质剪切波传播速度(SWV)并进行比较,分析CKD患者肾实质SWV测值与临床分期、肾小球滤过率(GFR)的相关性。结果随着病程分期进展、肾功能损害加重,肾实质SWV值逐渐减小。CKD组各期与对照组SWV的比较,对照组与CKD1期组(P=0.061)、CKD2期组与CKD3期组(P=0.238)、CKD4期组与CKD5期组(P=0.624)之间差异无统计学意义(P>0.05),余各组两两比较,SWV值差异均具有统计学意义(P<0.05);肾实质SWV值与CKD临床分期呈负显性相关(r=-0.65,P<0.001),即CKD分期越高,SWV值越低;SWV值与GFR呈正相关关系(r=0.73,P<0.001),即GFR越低,SWV值越低。结论 VTQ技术可以无创地反映肾实质的弹性变化,评价CKD患者肾功能损害程度,为CKD的临床分期提供了一种新的定量评估指标。Objective To investigate the application value of virtual touch tissue quantification(VTQ)technique in staging of chronic kidney disease(CKD).Methods VTQ examination was performed on a random sample of96CKD patients at different clinical stages and45healthy volunteers(control group).Shear wave velocity(SWV)of renal parenchyma was measured and compared.Correlation between SWV and clinical stage and glomerular filtration rate(GFR)of CKD patients was analyzed.Results The SWV value of renal parenchyma decreased gradually,along with the progression of CKD disease and impairment of renal function.There was no significant difference(P>0.05)between the control group and the CKD1group(P=0.061),the CKD2group and the CKD3group(P=0.238),and the CKD4group and the CKD5group(P=0.624).The difference of SWV between the remaining groups was statistically significant(P<0.05).SWV of renal parenchyma was negatively correlated with CKD stage(r=-0.65,P<0.001),which means the higher the stage of CKD was associated with the lower the SWV.SWV was positively correlated with GFR(r=0.73,P<0.001),which means the lower the GFR was related to the lower the SWV.Conclusion VTQ technique can noninvasively reflect the changes of renal parenchyma elasticity and evaluate the impairment of renal function in CKD patients.It can provide a new quantitative evaluation index for the clinical staging of CKD.
