血液灌流与血液透析联合治疗慢性肾衰竭患者的疗效及对其临床指标的影响

摘　　要：目的研究血液灌流联合血液透析对慢性肾衰竭的疗效及对其临床指标的影响。方法选取2015年3月~2016年9月我院收治的84例慢性肾衰竭患者。根据随机数字表法分成观察组与对照组各42例。对照组予以血液透析滤过治疗,观察组予以血液灌流联合血液透析治疗。对所有患者进行为期3个月的随访,分别对比两组患者治疗前后肾功能、炎性因子、血磷、血钙与甲状腺旁激素水平变化情况以及并发症发生情况。结果治疗后观察组BUN、Cr水平分别为(11.0±4.7)mmol/L、(601.3±193.5)μmol/L,均低于对照组的(13.7±5.3)mmol/L、(691.7±194.9)μmol/L。治疗3个月后观察组的IL-6、IL-8水平分别为(184.2±107.3)ng/L、(201.6±28.1)ng/L,均低于对照组的(291.4±108.2)ng/L、(254.6±35.7)ng/L。治疗1个月以及3个月后观察组的血磷、甲状旁腺激素水平均低于对照组,差异有统计学意义(P<0.05)。结论血液灌流联合血液透析治疗慢性肾衰竭的疗效显著,可显著改善患者炎症反应,调节血磷以及甲状旁腺激素水平。Objective To study the curative effect of patients with chronic renal failure and the influence of clinical indicators treated by blood perfusion combined with hemodialysis.Methods Selected84patients with chronic renal failure from Mar2015to Sep2016in our hospital.According to random number table method,they were divided into observation group and control group with42cases in each.Control group was given hemodialysis filtration treatment,observation group was given blood perfusion combined with hemodialysis treatment.For three months of follow-up of all patients,respectively compared the level changes of the two groups before and after treatment in patients with renal function and inflammatory factor,blood calcium,blood phosphorus and beside the thyroid hormone and complications.Results The level of BUN and Cr was respectively(11.0±4.7)mmol/L,(601.3±193.5)μmol/L in observation group after treatment,which was lower than control group(13.7±5.3)mmol/L,(691.7±194.9)μmol/L.The level of IL-6,IL8in observation group after treatment was respectively(184.2±107.3)ng/L,(201.6±28.1)ng/L,which was lower than the control group(291.4±108.2)ng/L,(254.6±35.7)ng/L.1month and3months after treatment,the blood of the observation group phosphorus,parathyroid hormone levels were lower than the control group.There were significant differences between the two groups(P<0.05).Conclusion Blood perfusion combined with hemodialysis has curative effect on chronic renal failure,it can significantly improve inflammation,adjust blood phosphorus and thyroid hormone levels.
