摘　　要：目的:探讨对脑卒中患者实行"1+N"4C延续护理效果。方法:由责任护士组成多学科团队对脑卒中患者实施以循证为基础"4C"及有目的有计划有评价的延续护理。结果:观察组出院1个月的MBI评分显著高于对照组,差异有统计学意义(P<0.05)。观察组出院3、6个月的MBI评分显著高于对照组,差异有统计学意义(P<0.01)。观察组的依存性显著高于对照组,差异有统计学意义(P<0.01)。观察组患者的复发率低于对照组,差异有统计学意义(P<0.05)。结论:"1+N"4C模式能使脑卒中患者自觉采纳健康的行为方式,降低再发病的危险因素,复发率得到有效控制,减轻患者家庭的经济负担,促进社会功能的回归。Objective:To discussed the effect of '1+N' 4C in the extended care of stroke patients. Meth-ods: Led by the responsible nurses, cooperating with other experts from other disciplines, carried out a purposeful,planned and evaluable extended care plan for strokepatients in observation group with the evidence-based '4C'.Results: The score of MBI discharged from hospital 1 month in the observation group was significantlyhigher thanthat in the control group, with significant difference(P < 0.05). The score of MBI discharged from hospital 3,6month in the observation group was significantly higher than that in the control group, with significant difference(P < 0.01). The dependence of the observation group was significantly higher than that of the control group,withsignificant difference(P < 0.01). The relapse rate of the observation group was lower than that of the control group,with significant difference(P < 0.01). Conclusions: '1+N' 4C help patients reduces the risk of being sick again byadopting healthy habits, which can effectively control the relapse rate and relieve the financial burden of patientsfamilies, and encourage patients regain the social function.
