摘 要:目的探讨果糖胺(FA)与餐后高血糖和血糖变异的相关性。方法回顾性分析2012年6月至2014年6月在聊城市第二人民医院内分泌科就诊的30例2型糖尿病(T2DM)患者(病例组)和在门诊体检的30例健康人(对照组)的临床资料。所有研究对象均签署知情同意书,符合医学伦理学规定。病例组中,男14例,女16例;平均年龄(55±9)岁;对照组中,男13例,女17例;平均年龄(56±10)岁。比较两组研究对象在空腹状态下的糖化血红蛋白(HbA1c)、FA、血肌酐、血清白蛋白和尿白蛋白排泄率水平;同时,比较动脉血糖仪反映的餐后血糖、平均餐后最高血糖(MPMG)、平均血糖标准差(SDBG)、平均每日高于10 mmol/L的曲线下面积(AUC-10)和平均血糖波动幅度(MAGE)等指标水平。两组资料的比较采用t检验或χ~2检验,两组研究对象的FA与24 h平均血糖、SDBG、MPMG、AUC-10和MAGE之间的相关性分析采用Pearson相关分析法。结果病例组患者的FA、SDBG、MPMG和MAGE水平分别为(3.1±1.2)(1.8±0.6)(10.4±1.4)和(4.2±1.5)mmol/L,明显高于对照组研究对象的(2.2±0.5)(1.2±0.3)(7.4±1.2)和(3.1±1.4)mmol/L(P<0.05)。对照组研究对象的FA、HbA1c与24 h平均血糖、SDBG、MPMG、AUC-10和MAGE均呈正相关关系(P<0.05);病例组患者的FA与24 h平均血糖、MPMG、AUC-10和MAGE呈正相关关系(P<0.05),而HbA1c只与24 h平均血糖呈正相关关系(P<0.05)。结论对于HbA1c达标的T2DM患者而言,FA与反映餐后血糖和血糖变异性的指标呈正相关关系,是反映餐后高血糖和血糖变异性的有效指标。Objective To investigate the correlation between fructosamine(FA)and postprandialhyperglycemia and glycemic variability.Methods Clinical data of30type2diabetes mellitus(T2DM)patients(the patient group)treated in the Endocrinology Department and30healthy subjects(the controlgroup)undergoing physical examination in the Outpatient Clinic of the Second People’s Hospital ofLiaocheng from June2012to June2014were retrospectively studied.The informed consents of all thesubjects were obtained and the local ethical committee approval had been received.In the patient group,14were males and16were females with an average age of(55±9)years old;and in the control group,13were males and17were females with an average age of(56±10)years old.The indexes of glycosylatedhemoglobin A1c(HbA1c),FA,serum creatinine and serum albumin in the fasting state and urinary albuminexcretion rate were compared between the two groups.In the meanwhile,the indexes of postprandial bloodglucose,mean postprandial maximum glucose(MPMG),standard deviation of blood glucose(SDBG),area under the curve of level above10mmol/L(AUC-10)and mean amplitude of glycemic excursions(MAGE)showed by arterial glucometer were also compared.The comparisons between the two groups were conductedusing t test orχ2test,and the correlation analyses between FA and24-hour average blood glucose,SDBG,MPMG,AUC-10and MAGE were conducted using Pearson Correlation Analysis.Results The levels ofFA,SDBG,MPMG and MAGE in the patient group were(3.1±1.2)(1.8±0.6)(10.4±1.4)and(4.2±1.5)mmol/Lrespectively,which were significantly higher than(2.2±0.5)(1.2±0.3)(7.4±1.2)and(3.1±1.4)mmol/L ofthose subjects in the control group(P<0.05).FA and HbA1c of subjects in the control group were positivelycorrelated with the24-hour average blood glucose,SDBG,MPMG,AUC-10and MAGE(P<0.05).FA of thesubjects in the patient group was positively correlated with the24-hour average blood glucose,MPMG,AUC-10and MAGE(P<0.05),but HbA1c was only positively correlated with the24-hour average blood glucose(P<0.0
