摘　　要：目的评价疾病管理在军事飞行人员高血压患者中的应用效果,为建立规范化军事飞行人员高血压的疾病管理模式提供科学依据。方法以54例军事飞行人员高血压患者为研究对象,通过建立档案、健康教育、个体化非药物治疗、个体化药物治疗、随访管理等措施进行疾病管理。采用自身对照研究方法比较疾病管理前和1年后患者动态血压监测(ABPM)、生活方式、症状自评量表(SCL-90)评分、超重肥胖率、血脂异常率的变化。结果疾病管理1年后,患者24 h、日间、夜间收缩压和舒张压均值、吸烟、过度饮酒、高钠饮食、缺乏运动等不良生活方式比例,SCL-90量表总分和躯体化、强迫、人际关系敏感、抑郁、焦虑因子分,以及超重肥胖率、血脂异常率均低于管理前(P<0.05)。结论对军事飞行人员高血压患者进行以建立档案、健康教育、个体化非药物治疗、个体化药物治疗、随访管理为一体化的疾病管理措施,能有效降低血压水平和高血压的相关危险因素,具有可行性和有效性。Objective To evaluate the effect of disease management of hypertension patients among military pilots in order to provide data for the establishment of a standard disease management model for hypertension among military pilots.Methods Fifty-four military pilots with hypertension were selected as subjects.Diseae management was implemented by means of archives,health education,individualized non-drug therapy,individualized drug therapy and follow-up.By adopting the method of seK control study,changes in ambulatory blood pressure monitoring(ABPM),lifestyles,symptom self-assessment scale(SGL-90)scores,the rate of overweight or obesity and dyslipidemia after a yearns disease management were compared.Results One year after disease management,the24h,daytime and nighttime systolic blood pressure and mean diastolic blood pressure,such unhealthy lifestyles as smoking,excessive drinking,high-sodium diet,and lack of exercise,and total score of SCL-90scale and somatization,OCD,interpersonal sensitivity,depression,anxiety factor scores,the rate of overweight or obesity,and the rate of dyslipidemia declined(P<0.05).Conclusion Such disease management measures as archives,health education,individualized drug therapy and individualized drug therapy,and follow-up not only can effectively reduce blood pressure levels and related risk factors for hypertension,but are feasible and effective.
