Observation on the clinical effect of three-dimensional positioning combined with quantitative gasket reset technology in the treatment of patients with fracture of tibial plateau

摘  要:目的:鉴于胫骨平台骨折传统手术方式存在局限性,研制一套简单易行的器械用于治疗SchatzkerⅡ、Ⅲ型胫骨平台骨折。方法:收治SchatzkerⅡ、Ⅲ型胫骨平台骨折患者30例。对照组16例采用传统切开直视观察关节面的手术方法,治疗组14例采用改良手术方式。结果:2种手术方式在手术时间、术后出血、切口长度、术后疼痛以及术后6个月Rasmussen膝关节功能评分等方面比较,差异具有统计学意义(P<0.05)。结论:改良后的手术方法在治疗胫骨平台骨折SchatzkerⅡ、Ⅲ型的患者时,能明显减少手术时间、切口长度、术后出血量以及术后1天疼痛程度,术后6个月膝关节功能恢复情况不差于传统手术方式,证明改良的手术方式是治疗胫骨平台骨折SchatzkerⅡ、Ⅲ型患者的安全、有效的方法。Objective:In view of the limitations of the traditional surgical approach for tibial plateau fractures,a simple instrument for the treatment of tibial plateau fractures of SchatzkerⅡandⅢtype is developed.Methods:30patients with fracture of tibial plateau of SchatzkerⅡandⅢtype were selected.In the control group(16cases),the traditional incision was used to observe the operation of the articular surface.The treatment group(14cases)underwent modified operation.Results:There were significant differences in the operation time,postoperative bleeding,incision length,postoperative pain,and Rasmussen knee function score in6months after operation,and the difference was statistically significant between methods(P<0.05).Conclusion:The modified method of operation can obviously reduce the operation time,incision length,postoperative bleeding volume and the degree of pain after1day in the treatment of patients with fracture of tibial plateau of SchatzkerⅡandⅢtype.The functional recovery of knee joint was not worse than that of traditional operation at6months after operation.It was proved that the modified operation was a safe and effective method for the treatment of tibial plateau fractures of SchatzkerⅡandⅢpatients.
