嗜酸性粒细胞减少和异常淋巴细胞的检出率升高在诊断恙虫病中的意义

摘　　要：目的探讨嗜酸性粒细胞减少和异常淋巴细胞的检出率升高对恙虫病的临床诊断意义。方法选择2015年6月~2016年6月来我院已明确诊断的40例恙虫病患者作为观察组,另选择40例健康者作为对照组,治疗前分别记录两组嗜酸性粒细胞水平和减少率,以及异常淋巴细胞水平和检出率,观察组经多西环素治疗5~10d后,复查嗜酸性粒细胞水平和减少率,以及异常淋巴细胞水平和检出率。结果治疗前观察组嗜酸性粒细胞减少率为87.50%(35/40),异常淋巴细胞检出率为92.50%(37/40),嗜酸性粒细胞计数显著低于对照组(P<0.05),异常淋巴细胞百分比显著高于对照组,差异有统计学意义(P<0.05)。治疗后观察组嗜酸性粒细胞计数显著高于治疗前(P<0.05),且减少率、异常淋巴细胞百分比和检出率均显著低于治疗前,差异有统计学意义(P<0.05)。结论嗜酸性粒细胞减少和异常淋巴细胞的检出率升高对恙虫病有重要诊断意义,检测手段方便快捷,值得临床推广。Objective To explore the clinical diagnosis significance of eosinophilia and abnormal lymphocytedetection rate in scrub typhus.Methods40cases of diagnosed scrub typhus in our hospital from June2015toJune2016were selected as observation group.Another40healthy subjects were chosen as the control group.Beforetreatment,the levels of eosinophils and the rate of reduction,the levels of abnormal lymphocytes and the rate ofdetection were recorded in the two groups.After treatment with doxycycline for5-10d in the observation group,thelevels of eosinophils and the rate of reduction,the levels of abnormal lymphocytes and the rate of detection wereexamined.Results Before the treatment,the rate of eosinophils reduction in the observation group was87.50%(35/40),the detection rate of abnormal lymphocytes was92.50%(37/40),eosinophil count was significantly lower thanthat in the control group(P<0.05),the percentage of abnormal lymphocyte was significantly higher than that of thecontrol group,and the difference had statistical significance(P<0.05).After the treatment,the eosinophil count in theobservation group was significantly higher than that before treatment(P<0.05),the rate of reduction,the percentageof abnormal lymphocyte and the detection rate were significantly lower than those before treatment,and the differencehad statistical significance(P<0.05).Conclusion Eosinophilia and abnormal lymphocyte detection rate hasimportant diagnostic significance for tsutsugamushi disease.Detection means are convenient and fast.It is worthy ofclinical promotion.
