心电图联合超声心动图对致心律失常性右室心肌病的诊断价值

摘　　要：目的探讨心电图联合超声心动图对致心律失常性右室心肌病的诊断价值。方法选择2014年2月至~2016年3月间我院诊治的致心律失常性右室心肌病患者33例作为观察对象,33例患者分别进行心电图和超声心动图检查,对比分析两种检查结果,探讨心电图联合超声心动图对致心律失常性右室心肌病的诊断价值。结果 33例致心律失常性右室心肌病患者中静息心电图显示,24例(72.73%)患者电轴右偏,QRS波增宽>110ms者22例(66.67%),T波倒置21例(63.64%),Epsilon波12例(36.36%);超声心动图ARVC患者主要表现为右室增大占90.91%,右室肌小梁紊乱占75.76%,右室流出道增宽占66.67%,右室壁弥漫性变薄占66.67%,右房增大占51.52%,其次表现为束回声增强占30.30%,室壁瘤形成占15.15%,右室壁局限性变薄占11.11%。结论心电图联合超声心动图检查能提高致心律失常性右室心肌病诊断准确性。值得临床推广采用。Objective To explore the diagnostic value of electrocardiogram combined with echocardiographyin arrhythmogenic right ventricular cardiomyopathy.Methods33patients with arrhythmogenic right ventricularcardiomyopathy diagnosed and treated in our hospital from February2014and March2016were selected as theobservation object.ECG and echocardiogram were performed on the33patients,respectively.Two inspectionresults were compared and analyzed.Diagnostic value of electrocardiogram combined with echocardiography inarrhythmogenic right ventricular cardiomyopathy were explored.Results Resting electrocardiographic findings in33patients with arrhythmogenic right ventricular cardiomyopathy showed24cases(72.73%)patients with right axisdeviation,22cases of QRS is wide>110MS(66.67%),T wave inversion in21cases(63.64%),12cases of Epsilonwave(36.36%).Echocardiography in ARVC patients mainly showed enlargement of the right ventricle,accounting for90.91%,the proportion of right ventricular muscle trabeculae was75.76%,and that of right ventricular outflow tractwas66.67%,diffuse thinning of the right ventricular wall accounted for66.67%,and right atrial enlargement accountedfor51.52%,followed by beam echo enhancement,accounting for30.30%,the left ventricular aneurysm formationaccounted for15.15%,right ventricular wall localized thinning accounted for11.11%.Conclusion ECG combinedwith echocardiography can improve the diagnostic accuracy of arrhythmogenic right ventricular cardiomyopathy.It isworthy of clinical application.
