内镜及腹腔镜治疗胰腺假性囊肿临床分析

摘　　要：目的 :分析内镜及腹腔镜治疗胰腺假性囊肿(Pancreatic pseudocyst,PPC)的疗效与安全性,总结治疗方案的选择依据。方法 :按照治疗方法,对比开腹组(n=74)、腹腔镜组(n=48)、内镜组(n=63)三组围术期指标、并发症发生情况及恢复情况。结果 :内镜组治疗时间、术中出血量、住院时间、术后排气时间、术后1 d疼痛评分、术后1 d恶心评分、术后1 d呕吐低于腹腔镜组,腹腔镜组上述指标低于开腹组,差异有统计学意义(P<0.05),三组患者治疗成功率、治疗有效率、复发率比较,差异无统计学意义(P>0.05)。开腹组并发症发生率高于腹腔镜组、内镜组,差异有统计学意义(P<0.05),腹腔镜组、内镜组并发症发生率比较,差异无统计学意义(P>0.05)。结论:内镜及腹腔镜治疗PPC疗效肯定且较开腹安全性更佳,应当区分适应征,选择合理的治疗方案。Objective:The objective of this study was to analyze the effectiveness and the safety of endoscopic and laparoscopic on the treatment of pancreatic pseudocyst and summarize the basis to choose the treatment.Methods:The index during perioperative period,implication incidence and recovery condition of laparotomy group(n=74),laparoscopic group(n=48)and endoscopic(n=63)were compared.Results:The treatment time,bleeding volume,length of stay,postoperative exhaust time,the pain score1d after surgery and nausea score1d after surgery were lower than that of laparoscopic group.All these index of laparoscopic group were lower than that of laparotomy group and the difference was statistically significant(P<0.05).The difference of success rate,effect rate and recurrence rate between these three groups was not statistically significant(P>0.05).The implication incidence rate of laparotomy group was higher than that of endoscopic and laparoscopic groups and the difference was statistically significant(P<0.05).The difference of implication incidence rate between endoscopic and laparoscopic groups was not statistically significant(P>0.05).Conclusions:The treatment of endoscopic and laparoscopic therapy on pancreatic pseudocyst is safer and better than laparotomy.Therefore,we should differentiate the indication and choose appropriate therapy.
