摘 要:目的探讨血清白介素-1β(IL-1β)及白介素-6(IL-6)在儿童高尿酸血症(HUA)发病中的作用及意义。方法选择HUA患儿71例,按是否有关节红肿热痛及活动障碍病史分为有关节症状组(GA组,n=28)及无关节症状组(NGA组,n=43);同时选取71例健康体检儿童和腹股沟斜疝修补术患儿为对照组。采用双抗体夹心酶联免疫吸附法(ELISA)检测所有患儿血清IL-1β、IL-6水平。结果 HUA组患儿血清IL-1β、IL-6浓度高于正常对照组,差异均有统计学意义(P均<0.05);GA组急性期血清IL-1β、IL-6浓度高于缓解期、NGA组及正常对照组,GA组缓解期和NGA组血清IL-1β、IL-6浓度高于正常对照组,差异均有统计学意义(P均<0.05)。患儿血清IL-1β、IL-6浓度与血白细胞计数、中性粒细胞、单核细胞、血尿酸、血沉、C反应蛋白、尿素氮、肌酐水平呈显著正相关(P均<0.05)),与血三酰甘油、总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇水平无相关性(P均>0.05)。结论 IL-1β、IL-6在HUA患儿的炎症反应机制中发挥重要作用。Objective To explore the significance of interleukin(IL)-1βand IL-6in serum of children with hyperuricemia(HUA).Methods142children including71children with HUA(HUA group)and71control children(control group),healthyand inguinal hernia children were selected as control group.71HUA children were subdivided into GA(gout attacks)group(n=28)and NGA(non-gout attacks)group(n=43)according to whether they had a history of acute gout attacks,including suddenmonoarthritis of rapid onset with intense pain and swelling or without.Enzyme-linked immunosorbent assay was used to measurethe level of IL-1βand IL-6in serum.Results Serum IL-1βand IL-6levels of HUA children were significantly higher than thoseof control group(all P<0.05).Serum IL-1βand IL-6levels of HUA children in GA group were significantly higher than thoseof NGA group(P<0.05).Serum IL-1βand IL-6levels of GA group in acute phase was significantly higher than those of HUAchildren in remission stage、NGA group and control group(P<0.05).Serum IL-1βand IL-6levels of GA group in remissionstage and NGA group was significantly higher than those of control group(P<0.05).There were no significant differencesbetween HUA children in remission stage and NGA group(P>0.05).The serum IL-1βand IL-6levels of HUA children werepositively correlated with WBC,neutrophils,monocytes,uric acid,ESR,CRP,BUN and Cr(all P<0.05),while not correlatedwith triglyceride,total cholesterol,LDL-C and HDL-C(all P<0.05).Conclusion IL-1βand IL-6play an important role in thepathogenesis of HUA in children.
