摘　　要：目的:探讨超声心动图(UCG)在冠心病早期诊断中的应用价值,为冠心病早期诊断提供新的参考方向。方法:选择本院2013年6月~2017年1月期间1000例疑似冠心病患者,全部患者均进行冠脉造影(CAG)和UCG检查,以CAG检查结果为金标准,分析UCG在冠心病早期诊断中的敏感性、特异性、阳性预测值、阴性预测值以及准确率。结果:UCG检查对心绞痛型冠心病的诊断敏感性明显低于心肌梗死型冠心病的敏感性,差异有统计学意义(P<0.05);UCG在病变支数为单支病变的敏感性53.07%、双支病变的敏感性66.67%、三支病变的敏感性76.49%,差异有统计学意义(P<0.05)。结论:UCG作为一种无创且操作简便的检查方式,在早期诊断冠心病为阳性的临床参考价值较高,对及时治疗具有重大意义;但UCG检查阴性预测值偏低,敏感性、特异性不高,因此临床使用仍应采取综合判断。Objective:To investigate the value of echocardiography(UCG)in the early diagnosis of coronary artery disease,provides a new reference direction for early diagnosis of coronary heart disease.Methods:Selected1000cases of suspected coronary heart disease in our hospital from June2013to January2017,all patients underwent coronary angiography(CAG)and UCG examination,the CAG test results as the gold standard,analyze the sensitivity,specificity,positive predictive value,negative predictive value and accuracy of UCG in the early diagnosis of coronary heart disease.Results:The sensitivity of UCG examination to the diagnosis of angina pectoris was significantly lower than that of myocardial infarction type coronary heart disease,the difference was statistically significant(P<0.05);The sensitivity of UCG to the number of lesions was53.07%for single vessel lesions,was66.67%for double branch lesions,was76.49%for the three lesions,the difference was statistically significant(P<0.05).Conclusion:UCG is a non-invasive and easy to operate method of examination,In the early diagnosis of coronary heart disease is positive,the clinical reference value is higher,and timely treatment is of great significance;However,the negative predictive value of UCG is low and the sensitivity and specificity are not high.Therefore,comprehensive judgment should be adopted in clinical practice.
