原发性高血压患者静息心率与代谢综合征的相关性研究

摘　　要：目的分析原发性血压患者静息心率增高与代谢综合征之间的相关性。方法入选2015年3月至2017年6月门诊及住院原发性高血压患者320例,根据RHR水平分为三组,RHR 1组(RHR<70次/分)52例,RHR 2组(RHR 70~85次/分)158例,RHR 3组(RHR>85次/分)110例,各组患者测定血压、腰围、空腹血糖(FIB)、糖负荷后2 h血糖、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、空腹胰岛素浓度(FINS)、胰岛素抵抗指数(IR)等指标。结果腰围、血压、FBG、糖负荷后2 h血糖、TC、TG、LDL-C、HDL-C、FINS、IR在不同RHR高血压患者各组间差异均有统计学意义(P<0.05),其中HDL-C随着RHR增加而降低,而其他指标随着RHR的增加逐渐升高。随RHR的增加,合并代谢异常率增加,差异有统计学意义(P<0.05)。RHR与腰围、FBG、糖负荷后2 h血糖、TC、TG、LDL-C、FINS、IR呈正相关(r值分别为0.36、0.61、0.45、0.28、0.45、0.41、0.36、0.54,P<0.05),与HDL-C呈负相关(r值为-0.41,P<0.05)。结论静息心率增快与高血压患者代谢紊乱程度相平行,可能参与了代谢综合征的病理过程。Objective To explore the relation between resting heart rate and metabolic syndrome in patients with essential hypertension.Methods320patients with essential hypertension were selected and divided into RHR1group(RHR<70beats per minute)(n=52),RHR2group(RHR70-85beats per minute)(n=158)and RHR3group(RHR>85beats per minute)(n=110).The waistline,bold pressure,FBG,plasma glucose of2hours post glucose-load,TC,TG,LDLC,HDLC,FINS,IR in the3groups were detected.Results There was significant difference of waistline,bold pressure,FBG,plasma glucose of2hours post glucose-load,TC,TG,LDLC,HDLC,FINS,IR in the3groups(P<0.05).HDLC level was reduced with increasing RHR and other indicators was rose with increasing RHR.The abnormal rate of metabolic syndrome was improved with increasing RHR(P<0.05).RHR had positive correlation with waistline,FBG,plasma glucose of2hours post glucose-load,TC,TG,LDLC,FINS and IR(r-values was0.36,0.61,0.45,0.28,0.45,0.41,0.36,0.54respectively,P<0.05).And RHR was negatively correlated with HDLC(r=-0.41,P<0.05).Conclusion RHR might be related with metabolic disorders in patients with essential hypertension.Therefore,More attention should be paid to improving many kinds of metabolism disorder and controlling heart rate.
