附加限制环的门腔静脉分流加断流和肝动脉强化灌注术治疗门静脉高压症的临床回顾性研究

摘　　要：目的:探讨附加限制环的门腔静脉分流加断流和肝动脉强化灌注术治疗门静脉高压症的疗效。方法:收治门脉高压患者50例,平分两组。对照组给予附加限制环的门腔静脉分流加断流术,强化组给予附加限制环的门腔静脉分流加断流和肝动脉强化灌注术。结果:两组术后门静脉血流量和门静脉直径均显著减小;强化组术后2周、1个月、6个月DHA及QHA与术前比较均显著增加,且明显高于对照组(P<0.05)。结论:施行附加限制环的门腔静脉分流加断流和肝动脉强化灌注术后,患者早期肝动脉血流升高,术后肝功恢复快,对肝功能有一定保护作用。Objective:To investigate the efficacy of portal hypertension treated by side to side portacaval shunt with the addition of restrictive ring plus periesophagogastric devascularization and enhancement of perfusion of hepatic artery.Methods:50patients with portal hypertension were selected.The patients in the control group were treated with portal hypertension treated by side to side portacaval shunt with the addition of restrictive ring plus periesophagogastric devascularization,and the intensive group was treated with portal hypertension treated by side to side portacaval shunt with the addition of restrictive ring plus periesophagogastric devascularization and enhancement of perfusion of hepatic artery.Results:The portal vein blood flow and portal vein diameter were significantly decreased in the two groups after operation;the DHA and QHA in the intensive group at2weeks,1month and6months were significantly higher than those of the control group after operation(P<0.05).Conclusion:After the portal hypertension treated by side to side portacaval shunt with the addition of restrictive ring plus periesophagogastric devascularization and enhancement of perfusion of hepatic artery treatment,the early hepatic blood flow was elevated,the liver function recovery after operation,and it has a protective effect on liver function.
