摘　　要：目的:探讨抗中性粒细胞胞浆抗体(ANCA)检测在系统性红斑狼疮(SLE)疾病活动性中的应用价值。方法:通过间接免疫荧光法(IIF)检测53例SLE患者血清中的ANCA,再通过酶联免疫吸附试验(ELASA)检测ANCA阳性患者血清中的特异性靶抗原蛋白酶3(PR3)和髓过氧化物酶(MPO)。检测SLE患者的病情活动性相关实验室指标。结果:53例SLE患者中检出ANCA阳性21例,阳性率为39.6%,核周型(pANCA)20例,胞浆型(c ANCA)1例。靶抗原均非MPO和PR3。与ANCA阴性组相比,ANCA阳性组在SLE疾病活动性中差异具有统计学意义(P<0.01),并且阳性组中患者抗ds DNA抗体阳性,补体(C3、C4)下降、血沉(ESR)增高、肾脏损伤的几率比阴性组显著增高,差异具有统计学意义(P<0.05)。结论:ANCA阳性在SLE疾病活动性中具有一定的应用价值,可作为判断病情活动性的指标之一。Objective:To study the value of ANCA in the detection of systemic lupus erythematosus disease activity.Methods:Theserum of53patients was tested by indirect immunofluorescence(IIF),and then the specific target antigen protein3(PR3)andmyeloperoxidase(MPO)in sera of patients with ANCA were detected by enzyme-linked immunosorbent assay(ELASA).Laboratoryindexes related to the disease activity of SLE patients were detected.Results:ANCA were detected in21patients with SLE:thepositive rate was39.6%,20had pANCA and one Cytoplasm antibody(cANCA).Target antigen was neither MPO nor PR3.Comparedwith ANCA negative group,there was a statistically significant difference in regard to the SLE disease activity in ANCA positive group(P<0.01).And the anti-dsDNA antibody was positive,complement(C3,C4)decreased,ESR increased and renal injury wassignificantly higher in the positive group than in the negative group.So the result was statistically significant(P<0.05).Conclusion:ANCA positive in the activity of SLE disease has a certain value and can be used as one of the indicators to determine the activity ofthe disease.
