针对性喉返神经减压术对甲状腺手术中喉返神经损伤的临床疗效观察

摘　　要：目的:观察针对性喉返神经减压术对甲状腺手术中喉返神经损伤的临床疗效。方法:收治行甲状腺切除术后发生喉返神经损伤患者132例,随机平分两组。观察组行针对性喉返神经减压术,对照组给予传统治疗方法。比较两组喉返神经恢复情况、嗓音声学参数及喉返神经诱发电位。结果:治疗后6个月,观察组喉返神经恢复有效率(96.97%)显著高于对照组(77.27%)(P<0.01);与治疗前相比,两组患者Shimmer、Jitter、NNE均降低(P<0.05或P<0.01),且观察组低于对照组(P<0.01);两组患者喉返神经诱发电位升高,且观察组高于对照组(P<0.01)。结论:针对性喉返神经减压术可促进甲状腺手术中喉返神经损伤的恢复,改善声带运动功能,提高患者声音质量。Objective:To observe the clinical effect of recurrent laryngeal nerve decompression on recurrent laryngeal nerve injury in thyroid surgery.Methods:132cases of patients with recurrent laryngeal nerve injury after thyroidectomy were selected.They were randomly divided into the two groups on average.The observation group was treated with targeted recurrent laryngeal nerve decompression,while the control group was given traditional treatment.The laryngeal nerve recovery,voice acoustic parameters and recurrent laryngeal nerve evoked potential were compared between groups.Results:After6months treatment,the effective rate of laryngeal nerve recovery in the observation group(96.97%)was significantly higher than that in the control group(77.27%)(P<0.01).Compared with before treatment,the Shimmer,Jitter,NNE of patients in the two groups were decreased(P<0.05or P<0.01),and the observation group was lower than the control group(P<0.01).The evoked potentials of the two groups increased,and the observation group was higher than the control group(P<0.01).Conclusion:The targeted recurrent laryngeal nerve decompression can promote the recovery of recurrent laryngeal nerve,improve the vocal cord movement function and improve the voice quality in thyroid surgery.
