肺内Castleman病1例并文献复习

摘　　要：目的 Castleman病(CD)是一种病因不明的、少见的淋巴组织增殖性疾病。肺内CD少见,为提高对该病的认识,通过病例分析肺内Castleman病特点,提高对此类疾病术前诊断可能性,选择合适的手术方式,减少术后出血。方法回顾性分析一例肺内Castleman病患者的临床资料并复习相关文献。结果本例患者行右肺上叶切除,术后病理诊断为肺内Castleman病。结论肺内Castleman病罕见,注意与肺癌鉴别,PCT-CT对术前诊断意义小,3D-CT血管造影对于辅助初步诊断这种罕见的肺内CD病例起到重要作用。选择肺叶切除术可减少术后出血。Objective Castleman’s disease(CD)is a rarely seen lymphoproliferative disorder with unknown etiology.Intrapulmonary lymph node involvement in CD is very rare.The aim of this study is to establish a correct preoperative diagnosisby analyzing the clinical features of intrapulmonary Castleman’s disease.Methods We retrospectively reviewed the medicalrecords of one patient with intrapulmonary CD and reviewed the literature of CD.Results To resection of right upper lobe,and the histopathological analysis revealed an intrapulmonary CD.Conclusion Intrapulmonary CD is exceedingly rare,whichshould be part of the differential diagnosis when an intrapulmonary mass with the features described in this report.The valueof the PET/CT in the preoperative diagnosis is low.We found that the angiographic features of3D-CT angiography were usefulfor diagnosing this rare case of intrapulmonary CD.
