控释地诺前列酮在晚期妊娠引产中的效果及安全性

摘　　要：目的探讨控释地诺前列酮在晚期妊娠引产中的效果及安全性。方法选取宁夏人民医院西夏分院2014年2月～2015年9月收治的60例晚期妊娠产妇，参照随机数表法分作对照组和试验组，每组30例，对照组予以缩宫素引产，试验组予以控释地诺前列酮引产，对比2组用药后宫颈Bishop评分变化、促宫颈成熟效果、引产情况及不良反应。结果用药后，试验组宫颈Bishop评分（8.35±0.93）分高于对照组（6.64±0.72）分（P＜0.05）；试验组促宫颈成熟总有效率96.67%（29/30）高于对照组80.00%（24/30），比较差异具有统计学意义（P＜0.05）；试验组（用药至临床时间、总产程）（14.89±1.57）h、（6.94±1.52）h优于对照组（21.48±2.35）h、（9.30±1.31）h（P＜0.05）；试验组阴道分娩率83.33%（25/30）高于对照组60.00%（18/30）（P＜0.05）；2组总不良反应率比较差异无统计学意义。结论控释地诺前列酮在晚期妊娠引产中的效果确切，阴道分娩率高，安全性高。Objective To study the effect and safety of controlled-release dinoprostone on the induced labor of late pregnancy.Methods60cases of late pregnancy puerpera were treated in our hospital from February2014to September2015,and divided into the control group andthe experimental group with reference to the stochastic indicator method,30cases in each group.The control group were given induced labor byoxytocin,and the experimental group were given induced labor by controlled-release dinoprostone,Changes of Bishop score,effects of promotingcervical mature,induced labor situation,and adverse reaction were compared between two groups.Results After medication,the cervical Bishopscore of the group(8.35±0.93)points were significantly higher than(6.64±0.72)of the control group(P<0.05).The total effective rate96.67%(29/30)of promoting cervical mature in the experimental group was significantly higher than80.00%(24/30)of the control group(P<0.05).Medicine to clinical time(14.89±1.57)h and total labor time(6.94±1.52)h of the experimental group were significantly better than(21.48±2.35)hand(9.30±1.31)h of the control group(P<0.05).The rate of delivery by vagina of the experimental group83.33%(25/30)was significantly higherthan60.00%(18/30)of the control group(P<0.05).There was no different on adverse reaction rate between two groups.Conclusion There wasgood effects of Controlled-release dinoprostone on the induced labor of later pregnancy,with high rate of delivery by vagina and better safety.
