2型糖尿病周围神经病变危险因素分析及前列腺素类似物的治疗效果观察

摘　　要：目的分析2型糖尿病周围神经病变的电生理检查，探讨其危险因素。方法选取2014年8月～2016年12月本院收治的2型糖尿病患者337例，行神经传导速度（NCV）和四肢皮肤交感反应(SSR)检测，根据结果分为DPN组218例和NDPN组119例，比较2组患者一般资料及实验室生化指标。采用Logistic多元回归分析DPN的危险因素。结果NCV联合SSR检出DPN的异常率高于单独应用NCV或SSR（P＜0.05）；DPN组和NDPN组在年龄、糖尿病病程、高血压病史、收缩压、2hFBG、HbA1c、FINS、2hINS、FC-P、2hC-P、ACR等差异有统计学意义（P＜0.05）；多因素logistic回归分析显示，年龄、糖尿病病程、2hFBG、HbA1c、ACR是DPN发生的独立危险因素。结论联合应用NCR与SSR可提高DPN的检出率；年龄大、糖尿病病程长、2hFBG、HbA1c、ACR高的2型糖尿病患者并发DPN的风险增加。Objective To explore the electrophysiological examination results and risk factors of type2diabetic peripheral neuropathy.Methods337patients with type2diabetes from August2014to December2016in the first people’s hospital in Shizuishan city were divided intoDPN group(n=218)and NPDN group(n=119)according to the results of NCV and SSR examinations.The general information and laboratorybiochemical indicators in the two groups were compared.Multivariate Logistic regression was used to analyze the risk factors of DPN.Results Thediagnosis rate of DPN detected by NCV combined with SSR was higher than that of NCV or SSR alone(P<0.05);There were significant differencesin age,duration of diabetes,history of hypertension,systolic blood pressure,2h FBG,HbA1c,FINS,2h INS,FC-P,2h FC-P,ACR between the DPNgroup and NPDN group(P<0.05);Logistic multivariable analysis showed that age,duration of diabetes,2h FBG,HbA1c,ACR were independentrisk factors for DPN.Conclusion It is beneficial to increase the diagnosis rate of DPN by NCV combined with SSR.There is a higher incidence rateof DPN type2diabetes patients with older grade,longer duration of diabetes,higher2h FBG,HbA1c and ACR.
