摘　　要：目的:评价CO_2点阵激光治疗化脓性肉芽肿的临床疗效及安全性。方法:将78例化脓性肉芽肿患者随机分成两组,CO_2点阵激光组41例,液氮冷冻组37例,治疗后3周和6个月观察疗效和安全性。结果:CO_2点阵激光组一次治愈率92.68%,总治愈率100%;液氮冷冻组一次治愈率56.76%,总治愈率94.59%,CO_2点阵激光组一次治愈率显著高于液氮冷冻组(P<0.05)。CO_2点阵激光组有8例出现不良反应,其中色素沉着5例,色素减退1例,0级瘢痕1例,++级瘢痕1例;液氮冷冻组19例出现不良反应,其中色素沉着7例,色素减退4例,0级瘢痕6例,+级瘢痕1例,++级瘢痕1例。两组色素沉着发生率及色素减退发生率差异无统计学意义(P>0.05),CO_2点阵激光组瘢痕发生率明显低于液氮冷冻组(P<0.05)。结论:与传统的液氮冷冻疗法相比,CO_2点阵激光治疗化脓性肉芽肿具有较好的疗效和较低的不良反应发生率。Objective To evaluate the therapeutic efficacy and safety of CO2fractional laser's treating pyogenic granuloma.Methods78pyogenic granuloma patients were recruited into this prospective study,which were further randomly divided into two groups:41patients were treated with CO2fractional laser and37patients were treated with liquid nitrogen freezing.We had assessed the efficacy and safety of two groups three weeks and six months later respectively.Results The CO2fractional laser group obtained a once curative rate of92.68%and a totally curative rate of100%.The liquid nitrogen freezing group obtained an once curative rate of56.76%and a totally curative rate of94.59%.The once curative rate of CO2fractional laser group was higher than that of liquid nitrogen freezing group(P<0.05).8patients of CO2fractional laser group appeared adverse reactions including hyperpigmentation(5/41),hypopigmentation(1/41),level0scar(1/41)and level2scar(1/41).While19patients of liquid nitrogen freezing group appeared adverse reactions which included hyperpigmentation(7/37),hypopigmentation(4/37),le vel0scar(6/37),level1scar(1/37)and level2scar(1/37).There is no significant differences between the two groups in terms of hyperpigmentation and hypopigmentation incidences(P>0.05).However,the CO2fractional laser group had a lower incidence of scar than the liquid nitrogen freezing group(P<0.05).Conclusion Compared to conventional liquid nitrogen freezing therapy,CO2fractional laser leads to better efficacy and fewer adverse effects for the treatment of pyogenic granuloma.
