摘 要:目的:探讨不同浓度的低浓度罗派卡因行腋路臂丛神经阻滞对前臂、手部手术的麻醉效果。方法:选取2016年1月—12月期间在我院行ASA I-II上肢手术治疗的患者60例作为本次研究对象,按照随机抽号方法将其分成三组,A组、B组、C组(n=20例)。A组患者行浓度为0.25%的罗哌卡因,B组患者行浓度为0.375%的罗哌卡因,C组患者行浓度为0.5%的罗哌卡因,对比分析3组患者行不同低浓度的罗哌卡因行腋路臂丛神经阻滞对患者前臂以及手部手术的麻醉效果,以及运动阻滞程度、运动阻滞时长的比较。结果:三组患者麻醉效果比较差异无统计学意义(P>0.05)。注射罗哌卡因30min后,B组和C组运动阻滞4级(不能屈腕)例数明显多于A组(P<0.05),B组和C组运动阻滞5级(不能活动手指)例数明显多于A组(P<0.05)。B组和C组运动阻滞恢复时间明显长于A组,P<0.05。结论:三种不同浓度的低浓度罗哌卡因麻醉药物,均可达到满意的麻醉效果,但0.25%的罗哌卡因可以在满意的麻醉效果下,保持一定的运动功能,对手术中前臂、手部功能的测定有很大帮助,同时可尽早恢复运动功能,更有利于肢体康复。Objective:To study the effect of different concentration of low concentration ropivacaine for axillary brachial plexus block on the forearmand hand surgery anesthesia.Methods:60cases of ASA I-II upper limb surgery patients from January to December2016in our hospital were randomlydivided into two groups,A group and B group(n=20cases).The patients of group A were treated with a concentration of0.25%ropivacaine,whilepatients in B group were treated with the concentration of0.375%ropivacaine,and patients in C group were treated with the concentration of0.5%ropivacaine.The anesthetic effect andtime/degree of motor block in3groups of patients were compared.Results:There was no significant differencebetween the three groups in anesthesia effect(P>0.05).After injection of ropivacaine for30min,there were4cases of motor block(no flexion wrist)in group B and group C.The number of cases was significantly more than that of group A(P<0.05),and the number of motion block and5stage(unable to move finger)in group B and group C was significantly higher than that in group A(P<0.05).The recovery time of motor block in group Band group C was longer than that in group A(P<0.05).Conclusion:All thesethree kinds of low concentration of ropivacaine can achieve satisfactoryanesthetic effect.While0.25%ropivacainecan also keep moving function,which is of great help forthe function of forearm and hand surgery,withshorter recovery time of motor function,and more conducive for rehabilitation.
