深静脉泵入高浓度氯化钾治疗ICU严重低钾血症患者的疗效及护理探讨

摘　　要：目的:探究ICU严重低钾血症患者,采用深静脉泵入高浓度氯化钾治疗的疗效及护理探讨。方法:选取时间段在2015年7月—2016年7月,在我院接收治疗的72例ICU严重低钾血症患者,采用的治疗方法是高浓度微泵深静脉补钾,将10%氯化钾,按照血清钾水平加等量0.9%NS在1~2h之内泵完。在进行补钾的过程中,对患者的尿量、心电图、肾功能、血清钾进行严密的监护。结果:血钾浓度在补钾前后进行对比,差异具有统计学意义(P<0.05),补钾速度和血钾浓度的改变呈线性关系,补钾后出现高钾血症者3例(占4.2%),补钾2~5h血钾等同于平常人有69例(占95.8%),100%是补钾有效率。结论:采用高浓度微泵深静脉补钾的方法治疗ICU危重早期低血钾患者,可满足部分ICU危重患者限制液体入量的要求,还可及时纠正术后低钾血症,方法安全有效,可提升ICU患者救活成功率,具有临床应用价值。Objective:To explore the effect and nursing of deep vein pump for high concentration potassium chloride treatment of severe hypokalemiain ICU.Methods:72cases of critically ill patients with severe hypokalemia from July2015to July2016in our hospital with severe hypokalemiawere treated with high concentration micro deep venous infusion.10%KCl and0.9%ns were infused through the pump for1~2h,and the serumpotassium levels were determined.During the process of potassium supplementation,the urine volume,ECG,renal function and serum potassiumwere carefully monitored.Results:The blood potassium concentration was compared before and after potassium supplementation,the difference wasstatistically significant(P<0.05);the change of potassium speed and potassium concentration showed a linear relationship,3cases(4.2%)of HIGHafter potassium supplement,69cases(95.8%)of potassium supplement of potassium supplement to normal(95.8%),and the efficiency of potassiumsupplement was100%.Conclusion:The method of using high concentration micro deep vein to treat patients with low serum potassium in ICUcritically ill patients can meet the requirement of limited liquid infusion in critical patients in intensive care unit,and can correct hypokalemia in time.The method is safe and effective,can improve the success rate of ICU patients,and has clinical application value.
