摘　　要：目的探讨初诊老年糖尿病肾病3期患者的贫血特点及影响因素。方法选取2015年8月—2017年7月在某三甲医院收治的初诊糖尿病肾病3期患者92例,其中年龄大于等于60岁患者48例为老年组,年龄小于60岁患者44例作为对照组(非老年组),比较两组患者贫血发生率,贫血程度及影响因素。结果老年糖尿病肾病3期患者贫血发病率高,贫血严重程度以中度贫血为主,与非老年糖尿病肾病3期组比较,差异有统计学意义(P<0.05),老年糖尿病肾病3期患者血清铁、白蛋白比非老年糖尿病肾病3期患者低,两组间的差异有统计学意义(P<0.05);铁蛋白、总铁结合力、身体质量指数,两组比较的差异无统计学意义(P>0.05)。结论老年糖尿病肾病3期患者更易发生贫血,且贫血程度较重,临床治疗时应给予充分重视。Objective To analyze the clinical characteristic and related factors of anemia in elderly patients with diabetic nephropathy in stage3in preliminary diagnosis.Methods A total of92patients with diabetic nephropathy in stage3who were enrolled hospital from August2015to July2017,including48cases with age greater than equal to60as elderly group and44cases with aged less than60years as the control group(non elderly group).The incidence of anemia,the degree of anemia and the influencing factors were compared between the two groups.Results The incidence of anemia was high in elderly patients with diabetic nephropathy in the3stage,the degree of anemia was mainly anemia,compared with the non elderly group,the difference was statistically significant(P<0.05).The serum iron and albumin of elderly patients with diabetic nephropathy were lower than those of no elderly patients with diabetic nephropathy,the difference between the two groups was statistically significant(P<0.05);Ferritin,total iron binding capacity and body mass index were not significantly different between the two groups(P>0.05).Conclusion Anemia ocurrs earlier and more serious in the diabetic nephropathy in stage3.Therefore,to identify anemia of diabetic nephropathy in stage3as early as possible,so that they will be timely treated.
