摘  要:目的评价1.32μm与1.06μm Nd:YAG激光对肝脏汽化切割与凝固止血能力。方法应用自主开发的双波长激光手术平台比较高能量1.32μm与1.06μm Nd:YAG混合输出激光对猪模型肝脏汽化切割与凝固止血能力,镜下观察与测量结果。结果在相同参数、剂量条件下,1.32μm激光照射产生的汽化宽度与深度值均大于1.06μm激光照射结果 ;两种激光照射创面均无活动性出血,1.32μm激光凝固层厚度比1.06μm激光凝固层厚度薄,但仍可达近3 mm。对1 mm及2 mm猪血管的切割,在肉眼均下无出血;镜下可见血管内长血栓,实验组在直径2 mm血管血栓长度为2~4 mm,对照组血栓长度2.3~4.9mm,均完全堵塞血管。结论 1.32μm波长激光的汽化切割能力强于1.06μm激光,对于2 mm以下的血管切割也能达到凝固止血的效果。Objective To analyze and evaluate1.32μm and1.06μm Nd:YAG laser ability for vaporization and coagulation in liver.Methods Self-developed dual wavelength high energy laser surgery platform was developed to compare1.32μm and1.06μm Nd:YAG laser on pig liver tissue cutting haemostasis.the results were observed and measured under microscope.Results The results were observed and measured under the same parameters,dose conditions.1.32μm laser were higher vaporization width and depth than1.06μm laser.The two laser irradiation was no active bleeding wounds.Coagulation in1.32μm condition was thinner than1.06μm laser with nearly3mm.For1mm and2mm swine vascular cutting,no bleeding under the naked eye.Under microscope were visible intravascular thrombus.Experimental group had vascular thrombosis length of2to4mm of2mm diameter and control group had thrombus length of2.3to4.9mm.The thrombosis completely blocked blood vessels.Conclusion1.32μm laser had more vaporization ability than1.06μm laser.It achieves hemostasis requirements for vessels less than2mm.
