皮瓣修复术联合超声清创治疗四肢较大创面的临床效果观察

摘　　要：目的:探讨皮瓣联合使用超声清创技术治疗四肢较大创面的效果。方法:选取医院收治的25例四肢创面行皮瓣修复患者,皮瓣修复前采用数表法随机将患者分为采用超声清创方法清创的观察组(13例)和采用外科清创的对照组(12例),比较两组创面的细菌清除率、疼痛程度、发热反应、皮瓣存活率、修复术前住院时间和修复术后感染情况。结果:观察组创面的细菌清除率和修复术后感染情况等方面明显优于对照组,两组比较差异有统计学意义(x^2=21.384,P<0.05)。两组清创治疗时间观察组少于对照组,两组比较差异有统计学意义(t=18.236,P<0.05)、两组皮瓣存活率均为100%,差异无统计学意义(x^2=98.457,P>0.05)。结论:超声清创技术对四肢较大创面能起到引流充分、清创彻底,减轻创面感染和改善血运,从而刺激肉芽组织快速生长;为修复创面起到了显著的效果,有利于缩短治愈时间,能够降低皮瓣修复术后感染率。Objective:To investigate the effect of combined use of skin flaps and ultrasonicdebridement in the treatment of large wounds of limbs.Methods:25patients with wound inlimbs who prepared to undergo prosthetics of skin flap were divided into observation group(13cases who received ultrasonic debridement)and control group(12cases who received routinedebridement of surgery)as random number table before the surgery.A series of indicatorsincluded the clearance rate of bacteria,pain degree,exothermic reaction,survival rate of skin flap,hospital stay timepre-prosthetics and the infection situation post-prosthetics were compared and analyzed.Results:The clearancerate of bacteria of wound and infection situation post-prosthetics of observation group were significantly better thanthat of control group(x2=21.384,P<0.05).The curative time of debridement of observation group was significantlylower than that of control group(t=18.236,P<0.05).Both of the two survival rates of skin flap of the two groupswere100%,and the differences of the two groups was no statistically significant(x2=98.457,P>0.05).Conclusion:The ultrasonic debridement technique can provide adequate drainage,complete debridement,and reduce woundinfection and improve blood transport.Therefore,it can stimulate the granulation tissue to rapidly grow.It has aremarkable effect on the repair of the wound,which can shorten the cure time and reduce the postoperative infectionrate of skin flap.
