摘　　要：目的观察右美托咪定对重度烧伤患者血清心肌肌钙蛋白I(cardiac troponin,cTnI)、肌酸激酶同工酶MB(creatine kinase isozyme-MB,CK-MB)及心型脂肪酸结合蛋白(heart fatty acid-binding protein,h-FABP)的影响,探讨其对重度烧伤患者损伤心肌的保护作用。方法选取本院2015年11月至2016年12月收治的120例择期拟行切痂植皮术治疗的重度烧伤患者为研究对象,按照入院顺序将其分为观察组和对照组,每组各60例。观察组患者麻醉诱导前10分钟静脉泵注右美托咪定0.6μg/kg,10分钟后改为0.2~0.5μg/(kg·h)维持泵注直至手术结束,对照组患者予以等容量生理盐水,注射方式同观察组。观察两组患者治疗后血清cTnI、CK-MB水平及h-FABP阳性率。结果术毕、术后12小时及术后24小时,观察组患者血清cTnI、CK-MB水平与h-FABP阳性率均明显低于对照组(P<0.05)。结论右美托咪定抑制cTnI、CK-MB及h-FABP的表达,显著减轻重度烧伤患者术后心肌细胞的损伤,且患者术后未出现明显不良反应,安全性较高。Objective To investigate the influence of Dexmedetomidine on cardiac troponin(cTnI),creatine kinase isozyme-MB(CK-MB)and heart fatty acid-binding protein(h-FABP)of severe burn patientsand protection on injured myocardium.Method120patients with severe burn in our hospital from November2015to December2016were selected and divided into two groups with60cases in each group.Patients in observation group were given intravenous infusion of Dexmedetomidine0.6μg/kg at10minutes before induction and then after10minutes the injection dose changed into0.2～0.5μg/(kg?h)maintain infusion until the end of surgery.Patients in control group were given an equal volume of saline injection and the injection method was similar with that of observation group.The serum levels of cTnI,CK-MB and h-FABP positive rate were observed in two groups after treatment.Result After surgery and12h,24h after surgery,the serum levels of cTnI and CK-MB and h-FABP positive rate of observation group were lower than those of control group(P<0.05).Conclusion Dexmedetomidine inhibits the expression of cTnI,CK-MB and h-FABP of severe burn patients,it can significant decrease myocardial cell injury and has no obvious adverse reactions.
