富血小板血浆联合同种异体骨移植治疗早期股骨头坏死

摘　　要：目的观察自体富血小板血浆(PRP)联合同种异体骨移植对早期非创伤性股骨头坏死干预性治疗的临床效果。方法回顾性分析2014年3月至2015年3月荆州市第二人民医院收治的18例(22髋)早期非创伤性股骨头坏死患者的临床资料,均采用髓芯减压联合PRP、同种异体骨移植进行治疗。根据Harris评分及影像学Ficat分期标准观察患者术后疼痛、功能改善及病情进展情况。结果随访时间12~24个月(平均18个月)。术后1年Harris评分明显高于术前(P<0.05),其中优9髋、良6髋、可5髋、差2髋,优良率68%(15/22);术后1年影像学Ficat分期提示3髋进展,余病例无明显加重表现,进展率14%(3/22)。结论自体PRP结合同种异体骨移植干预性治疗早期非创伤性股骨头坏死,临床效果及X线影像学表现满意,可作为保髋手术的策略之一。Objective To observe the intervention effects of autologous platelet rich-plasma(PRP)andallogenic bone transplantation on early non-traumatic osteonecrosis of femoral head(ONFH).Methods Clinicaldata of18patients(22hips)with early non-traumatic ONFH treated from March2014to March2015in theSecond People's Hospital of Jingzhou,were retrospectively analyzed.All patients were performed coredecompression combined with PRP and allogenic bone transplantation.According to Harris hip score andradiographic criteria of Ficat grading,pain relief and hip function improvement,as well as disease progressionwere evaluated after the surgery.Results The patients were followed up for12to24months(average,18months).After1year,Harris score was improved significantly compared to preoperative one(P<0.05),theexcellent and good rate was68%(excellent in9hips,good in6,fair in5and poor in2hips);X-ray imagingshowed progression in3hips,with the progression rate of14%(3/22).Conclusion Intervention treatment of PRP and allogenic bone grafting for early non-traumatic ONFH has good clinical effects and satisfactory X-rayimaging,which could be regarded as a strategy of hip preservation surgery.
