宫颈锥切术围手术期预防应用抗菌药物调查与分析

摘　　要：目的回顾性调查《抗菌药物临床应用指导原则(2015年版)》对本院宫颈锥切术围手术期预防性应用抗菌药物的干预情况。方法抽取本院(干预前)2013年6月~2014年12月、(干预后)2016年1月~2017年3月行单一宫颈锥切术各110例,调查分析围术期预防应用抗菌药物的种类、抗菌药物使用率,给药疗程、联合用药及术后感染情况。结果依据抗菌药物临床应用指导原则干预后,本院宫颈锥切术围术期预防性使用抗菌药物品种选择较为合理,第一代头孢菌素使用率由21.82%上升至34.55%,第三代头孢菌素使用率由12.73%下降至0.00%,抗菌药物使用时间24小时内病例数由32.72%上升至53.64%,抗菌药物使用时间48小时内病例数由53.64%下降至37.27%,两组比较差异有统计学意义(P<0.05)。结论本院根据《抗菌药物临床应用指导原则(2015年版)》干预临床抗菌药物管理,有效限制第三代头孢菌素用于围手术期预防用药,减少抗菌药物临床应用时间,并未增加感染的风险。Objective To investigate the effect of Guiding principles of clinical application of antibiotics(2015Edition)of perioperative prophylacticuse of antibiotics in cervical conization in our hospital.Methods The date of110cases of single cervical conization from our hospital prior tothe intervention during June2013to December2014and after the intervention from January2016to March2017was used.Different kinds,appliedrate of antibacterial drugs as well as the course of treatment,combined drugs and postoperative infection was analysed.Results Based on the guidingprinciples of clinical application of antibacterial drugs,the selection of antibiotics prophylactic used on perioperative cervical conization in ourhospital was comparatively reasonable.The utilization rate of the first generation of cephalosporin increased from21.82%to34.55%as well as thethird generation of cephalosporin decreased from12.73%to0.00%.Meanwhile,the case of prophylactic use of antibiotics within24hours increasedfrom32.72%to53.64%,within48hours decreased from53.64%to37.27%.The result was statistically difference(P<0.05).Conclusion Interventionin management of clinical antibiotics according to Guiding principles of clinical application of antibiotics(2015Edition)was observed in ourhospital.The third generation of cephalosporins used in perioperative prophylaxis was effectively limited,the time of clinical application of antibioticswas reduced without increasing the risk of infection.
