无创通气在全身麻醉恢复期辅助呼吸的应用

摘　　要：目的分析无创通气在全身麻醉恢复期辅助呼吸的临床应用效果.方法选取2012年10月至2016年12月曲靖市第二人民医院麻醉科共1 380例全身麻醉恢复期留置口咽通气道患者作为研究对象,按照随机数字表将其分为:螺纹管面罩给氧组(n=690)及无创通气组(n=690),螺纹管面罩给氧组采用四头带面罩给氧,无创通气组采用无创通气,对比2组患者术后15 min HR、BP、SpO_2、RR、不良反应、满意度以及清醒时间.结果无创通气给氧组的术后15 min HR、BP、SPO_2、RR、上呼吸道梗阻、呼吸困难、无助恐惧感、安全性、患者舒适(自然清醒)感、满意度明显好于螺纹管面罩给氧组(P<0.05);无创通气给氧组患者的清醒时间为(10.15±2.26)min,螺纹管面罩给氧组患者的清醒时间为(16.13±3.73)min(P<0.05).结论在全身麻醉恢复期,与螺纹管面罩给氧相比,采用无创通气能够大大提高患者麻醉恢复期呼吸及循环参数、安全性、舒适感及满意度,进一步缩短清醒时间,值得推广.Objective To analyze the clinical application effect of noninvasive ventilation in assistingrespiration during convalescence of general anesthesia.Methods We selected1380cases of patients with retainingoropharynx air passage during convalescence of general anesthesia as our object,fromOct,2012to Sep,2016atthe anesthesiology department in the2nd people's hospital of Qujing.Abiding by random number table,they weredivided into:facial mask with screwed conduit for oxygen inhalation group(n=690)and noninvasive ventilationgroup(n=690).In the former group,the oxygen was inhaled by facial mask with four tailed bandage,while thenoninvasive ventilation group using noninvasive ventilation.The HR,BP,SPO2,RR,ADR and satisfactionand sobering time were compared between the patients of two groups15min after operation.Results Innoninvasive ventilation for oxygen inhalation group,HR,BP,SPO2,RR,the upper respiratory tract obstruction,dyspnea,a sense of fear and helplessness,safety,sense of comfort(naturally sober),satisfaction wereobviously better than those of the facial mask with screwed conduit for oxygen inhalation group(P<0.05);thesobbing time of noninvasive ventilation for oxygen inhalation group was(10.15±2.26)min compared with the timeof another group(16.13±3.73)min(P<0.05).Conclusion During the convalescence of general anesthesia,compared to oxygen inhalation by facial mask with screwed conduit,the noninvasive ventilation can greatly improvethe respiratory and cycle parameters of patients as well as the safety,sense of comfort and satisfaction,and furthermore it can shorten the sobering time for patients,so it worth promotion.
