联合固定技术治疗髌骨粉碎性骨折的临床探讨

摘　　要：目的探讨克氏针、张力带、钢丝环扎、可吸收丝线缝合联合固定技术治疗髌骨粉碎性骨折的临床效果。方法回顾性分析2010年6月—2015年5月唐山市第二医院收治并成功随访的32例髌骨粉碎性骨折患者。其中男性20例,女性12例;平均年龄44.5岁(19~65岁);左侧10例,右侧22例。致伤原因:摔伤10例,道路交通伤15例,高处坠落伤7例。所有患者均采用切开复位克氏针、张力带、钢丝环扎、可吸收丝线缝合联合固定技术治疗。结果 32例术后均获得随访,时间12~28个月,平均19.5个月,所有骨折全部骨性愈合。按照B9stman膝关节评分标准评定,优16例,良12例,差4例;优良率87.5%。结论克氏针、张力带、钢丝环扎、可吸收丝线缝合联合固定技术治疗髌骨粉碎性骨折,固定牢固,简单实用,术后可早期行膝关节功能锻炼,减少术后并发症的发生,疗效满意。ObjectiveTo evaluate the effectiveness of combined fixation(kirschner wire,AO tension band,cerclage wire,absorbable suture)in the treatment of comminuted fracture of patella.MethodsA retrospective analysis was conducted in32patients with comminuted fracture of patella who were admitted into our hospital from Jun.2010to May2015There were20males and12females,with an average age of445years(range,19to65).Ten cases were injured on the left side,and22cases on the right side.Injury causes included10cases of falling,15cases of road traffic injury,and7cases of high falling.All patients were treated through open reduction and internal fixation with kirschner wire,AO tension band,cerclage wire and absorbable suture.ResultsAll of the32patients were followed up from12months to28months(on average of195months).All patients obtained good fracture union.The Bstman standard showed excellent in16cases,good in12cases and poor in4cases,with an excellent and good rate of875%.ConclusionThe combined fixation of kirschner wire,AO tension band,cerclage wire and absorbable suture in the treatment of patients with comminuted fracture of patella is a reliable fixation technique,which can obtain full knee motion,reduce postoperative complications and have good curative effect.
