联合检测血浆D二聚体、FDP、FM、ATⅢ含量对DIC的诊断价值

摘　　要：目的探讨联合检测血浆中D-二聚体(D-D)、纤维蛋白原降解产物(FDP)、纤维蛋白单体(FM)、抗凝血酶Ⅲ(ATⅢ)含量在弥漫性血管内凝血(DIC)诊断中的临床意义.方法选取40例早期DIC患者,40例DIC可疑患者,40例正常人(对照组),分别检测3组人群血浆中D-D,FDP,FM,ATⅢ的含量.结果早期DIC患者血浆中D-D,FDP,FM的含量明显高于DIC可疑患者和正常对照组,早期DIC患者血浆中ATⅢ的活性明显低于可疑患者和正常对照组,差异具有统计学意义(P<0.05),联合检测早期DIC患者血浆中D-D,FDP,FM,ATⅢ的含量可提高早期DIC诊断的灵敏度和特异性.结论通过联合检测血浆中D-D,FDP,FM,ATⅢ含量对早期DIC诊断具有临床指导意义.Objective To investigate the clinical significance of joint detection of D-dimer(D-D),fibrinogen degradation products(FDP),fibrin monomer(FM)and antithrombinⅢ(ATⅢ)contents in plasma for the diagnosis of disseminated intravascular coagulation(DIC).Method A total of40patients with early DIC,40DIC suspicious patients,and40cases of normal people(control group)were enrolled in this study.The plasma levels of D-D,FDP,FM and ATⅢof three groups were determined respectively.Results The contents of D-D,FDP,FM in plasma of the patients with early DIC were significantly higher than those of other two group,and the early activity of ATⅢof the patients was significantly lower(P<0.05).Combined detection of D-D,FDP,FM,ATⅢcan be used to improve the sensitivity and specificity for the diagnosis of early DIC.Conclusion Combined detection of plasma levels of D-D,FDP,FM and ATⅢhas important clinical value for the diagnosis of early DIC.
