摘 要:目的观察血浆冷沉淀治疗家兔感染性创面修复的效果,阐述血浆冷沉淀促进家兔感染性创面修复的作用机制。方法将健康的新西兰成年家兔12只,按照改良付小兵全层皮肤缺损法复制创伤感染性动物体表溃疡模型,分别予冷沉淀、康复新液、湿润烧伤膏和凡士林纱布换药,测定治疗后第3、7、12、17天创面微血管密度(MVD),创面愈合率和新生上皮覆盖率,创面转化生长因子β_1(TGF-β_1)表达情况。结果各组治疗第3天MVD均呈上升趋势,第3、7天冷沉淀组MVD与其他3组比较,差异有统计学意义(P<0.05),第12天后新生血管逐渐减少;冷沉淀组创面愈合率、新生上皮覆盖率与康复新液组、凡士林组比较,差异有统计学意义(P<0.05),冷沉淀组创面完全愈合时间与其他3组比较,差异有统计学意义(H=11.169,P=0.011),冷沉淀组缩短;冷沉淀组治疗早期TGF-β_1表达与其他3组比较差异有统计学意义(P<0.05),并在整个修复过程中维持高表达。结论冷沉淀可能通过上调创面TGF-β_1表达、加速创面MVD,达到提高创面新生上皮覆盖率,促进创面快速愈合的目的。Objective To observe the effect of plasma cryoprecipitate on the repair of rabbit infectivewounds,and then explore the mechanism of the cryoprecipitate in promoting the healing of rabbit infectivewounds.Methods Twelve healthy adult rabbits,according to the improved Fu Xiaobing full-thickness skin defectmethod,were made into wound infectious animal surface ulcer model,and then were treated respectively withthe cryoprecipitate,Kangfuxin solution,MEBO or Vaseline gauze dressing.On the3rd,7th,12th and17th day oftreatment,the microvascular density(MVD),healing rate,new epithelial coverage rate and TGF-β1expression of thewound were tested.Results On the3rd day of treatment,MVD rose in all the groups;on the3rd and7th day,MVDin the cryoprecipitate group was statistically different from that of the rest three groups(χ23d=15.151,χ27d=27.837;P=0.002and0.000);after the12th day,new blood vessels gradually reduced.The wound healing rate and the newepithelial coverage rate in the cryoprecipitate group were higher than those in the Kangfuxin solution group and theVaseline group(P<0.05).The wound healing time in the cryoprecipitate group was significantly shorter than thatin the other groups(H=11.169,P=0.011).TGF-β1expression in the cryoprecipitate group was higher than thatin the other three groups(χ23d=17.400,χ27d=21.969;P=0.001and0.000),and maintained high level during the entire repair process.Conclusions The cryoprecipitate can increase MVD of the wound,promote the new epithelialcoverage,and accelerate wound healing through up-regulation of TGF-β1expression in the wounds.
